Description
Akihiro Kanou is a central figure in the narrative, initially appearing as a benevolent physician but later revealed to be a mad scientist whose actions drive much of the conflict. He is a former employee of the Ghoul Forschung Gesellschaft (GFG) and the Commission of Counter Ghoul (CCG), who later becomes affiliated with the ghoul terrorist organization Aogiri Tree.

Kanou is an elderly man with grey hair, typically seen wearing a physician's white coat, shirt, and tie, though he later adopts more casual attire like a Hawaiian shirt. His outward demeanor is that of a kind-hearted doctor who disregards regulations if it means saving a patient's life. This facade, however, masks a profoundly disturbed and misanthropic personality. Kanou views humanity as weak and possessing no potential for the future, having lost all hope in a world populated by humans. He sees ghouls, and specifically artificial one-eyed ghouls, as the next step in human evolution, a "ray of hope" that can break the world free from its "birdcage". To achieve this vision, he shows no remorse for his test subjects, manipulating their loneliness and naivete and discarding them as mere stepping stones for his research. Despite his hatred for humanity, he can respect individuals for their abilities, such as Dr. Kouitsu Chigyou for refining the Quinx surgery.

A major turning point in Kanou's past is the death of his sickly mother shortly before he entered university, an event that reportedly changed him greatly. He graduated at the top of his class from Teihou University's School of Medicine before traveling to Germany for research with the GFG. He later joined the CCG, where his research into integrating ghoul capabilities into humans was terminated due to ethical concerns, leading to his resignation.

Kanou's most significant act is transforming Ken Kaneki into an artificial one-eyed ghoul. After Kaneki is mortally wounded by Rize Kamishiro, Kanou performs a kakuhou transplant surgery, using Rize's organs without permission and causing a major scandal. He refers to Kaneki as his "prototype" and "ultimate creation," and while he never intended for him to be more than a test, he becomes extremely pleased with his growth and later refers to him as his "beloved prototype". Kanou's role in the story expands as he creates other artificial one-eyed ghouls, including Kurona and Nashiro Yasuhisa, whom he manipulates by revealing the truth about their parents' deaths and who call him "papa". He is also responsible for the ghoulification of Seidou Takizawa, the only successful product from sixty-three investigator subjects, and of Koutarou Amon, who is designated a failure or "floppy". Later, he uses Rize as a captive donor, farming her kakuhou for further experiments.

Kanou has key relationships with several other characters. He considers Eto a crucial ally with similar goals regarding the "birdcage," and he frequently interacts with her within Aogiri Tree. He also works closely with Tatara, who provides him with facilities and test subjects. He is on good terms with the Clowns, who refer to him as a "good friend". In his professional capacity, he respects Dr. Chigyou's work on the Quinx surgery and hopes to converse with him. He holds a deep disdain for Rize, whom he treats as a mere resource, keeping her captive and in a state of suffering.

Throughout the series, Kanou's development sees him refining his ghoulification process, incorporating technologies like the Quinx's frame system to improve success rates. He operates from hidden labs, first in the suburbs of Tokyo and later on Rushima, where he continues his experiments even as he comes into conflict with his former subjects like Kurona.

In terms of abilities, Kanou possesses keen intellect and charisma, allowing him to persuade others to become his test subjects. His primary ability is his extensive medical expertise. Despite his disregard for ethics, he is a brilliant surgeon with astonishing knowledge of both human and ghoul anatomy. This allows him to perform the complex kakuhou transplant surgery and create Rc pathways within a human body, successfully converting several individuals into one-eyed ghouls. His research data is also highly valued and studied by the CCG.
