As the toll for seeing the Truth behind alchemy, she lost several of her internal organs, was rendered infertile, and suffered a permanent condition that caused her to periodically cough up blood. However, this harrowing experience also granted her the rare and powerful ability to perform alchemy without a transmutation circle, a skill she shares with only a few others who have committed the same taboo.

In terms of personality, Izumi is a woman of extremes. She is renowned for her incredibly strict and violent form of discipline, often beating the Elric brothers as punishment for mistakes or for joining the State Military, which she despises.
