Handsomely Masked Sweet Mask holds the position of A-Class Rank 1 professional hero within the Hero Association, a rank he maintains by choice. Beyond his hero duties, he is an extraordinarily famous model, actor, and singer, widely regarded as the most popular celebrity in the world. He also serves on the Hero Association's hero rank evaluation staff, giving him a direct voice in which heroes are promoted. Physically, Sweet Mask is a tall, lean young man with messy, shoulder-length light blue hair and yellow eyes, though his eyes are known to turn blood-red when he is extremely angered.

Sweet Mask's past reveals a profound transformation. He was born with a very ugly face, which he was deeply ashamed of, so he resolved to be beautiful on the inside by adhering to strict moral principles. He originally performed hero work wearing a mask to conceal his features, going by the hero name "Secret Mask". During a battle, an accident shattered his mask, exposing his true face to bystanders. Instead of jeers, he was met with cheers. Seeing his reflection in a puddle, he witnessed that his appearance had become strikingly beautiful. He realized that his intense hatred for his own ugliness had caused his body to transform, effectively turning him into a monster. This history explains much about his current personality and obsessions.

The personality of Sweet Mask is built on a stark duality. On the surface, he is charismatic, popular, and genuinely cares for the well-being of citizens, wishing happiness upon all living things. He believes heroes must be tough, strong, and beautiful at all times to ease the fears of the public, and he works hard to please his fans. However, beneath this charming exterior lies a merciless and ruthless nature. He harbors a burning hatred for anything he considers evil, which he believes must be swiftly and thoroughly exterminated. He has ignored orders from his superiors to capture enemies alive, instead executing them on the spot as he sees fit. This cruel streak extends to his fellow heroes; he views promotions to S-Class or A-Class as matters of public attention that affect his own reputation. He deliberately remains at A-Class Rank 1 to prevent any hero he deems unworthy from advancing to S-Class. He is deeply narcissistic about his philosophy that "beauty overwhelms power" and has little respect for heroes whose abilities or appearances he finds lacking.

Sweet Mask's motivations are driven by a twisted sense of justice and an obsession with aesthetics. He demands that heroes be perfect symbols of strength and beauty who can defeat evil quickly and elegantly. He shows interest in heroes like Genos, who possess both skill and a beautiful appeal. Conversely, he has contempt for those he believes are weak or fail to live up to his ideal. His role in the story often involves acting as a gatekeeper and a critic of the Hero Association's standards. He is deeply involved in the raid on the Monster Association, where his uncompromising attitude puts him at odds with other heroes. He questions the humanity of heroes like Zombieman due to their monstrous abilities and insists on leading the charge, believing himself superior to many S-Class heroes.

Key relationships define much of his story arc. Initially, he looks down on the protagonist Saitama, but after witnessing the hero's overwhelming power, he becomes obsessed. He recognizes Saitama as possessing the "true beauty of overwhelming strength," which shifts his focus from mere physical beauty to raw, absolute power. He also has a notable relationship with Genos, whom he congratulates on reaching S-Class while also expressing disappointment in his performance. After being brutally defeated by the monstrous Garou and witnessing Saitama's victory, his viewpoint changes, becoming more focused on the overwhelming power of the individual rather than beauty alone. This encounter forces him to confront the limits of his own strength and the nature of his own monstrous transformation, leading to a reevaluation of what it truly means to be a hero.

In terms of notable abilities, Sweet Mask possesses immense power that far exceeds his A-Class ranking. Many characters, including the S-Class hero Atomic Samurai, confirm that he is definitely worthy of S-Class. His physical abilities include immense speed and reflexes, allowing him to kill captured aliens before nearby S-Class heroes could react and evade attacks from Dragon-level monsters. He has incredible brute strength, able to lift and throw massive boulders with ease and destroy large structures with a few swipes. His endurance is remarkable; he has shown no pain upon being pierced through the chest and continued fighting normally.

One of his most distinctive traits is his abnormal body possessing regenerative capabilities. He has demonstrated the ability to reattach a severed arm, claiming a clean cut made this possible, and has regenerated a smashed face after a powerful punch. However, he does not seem capable of regrowing limbs entirely, requiring reattachment instead. In close combat, he is a master hand-to-hand fighter, employing a brutal and efficient style that eliminates opponents before they can retaliate. He also has high accuracy in throwing objects and possesses a mental resistance that allows him to completely resist mind-control abilities. Despite his tremendous power, Sweet Mask has a very peculiar and exploitable weakness: extreme ugliness paralyzes him, making him unable to move or fight back effectively, rendering him particularly vulnerable against hideous monsters.
