She is affiliated with the Cleaners and first appears when she treats the protagonist Rudo, who is in rough shape after a battle. Despite her advanced age, she remains an active and vital member of the community, well-known for her medical expertise.

Her personality is described as loud and unapologetic, with a notably sharp tongue. She is a no-nonsense individual who immediately puts Rudo in his place upon meeting him, showing she is not easily intimidated and has little patience for foolishness. One of her most distinctive traits is her tendency to scream constantly. This is not a simple personality quirk but rather a direct side effect of the intense research she has conducted throughout her career.

Alice Stilza is not just any healer; she is a pioneer in her field. She was the first person to ever study cures for body contamination, a significant and likely dangerous medical condition within the world of the story. Because of this groundbreaking work, she is widely recognized as the absolute best healer, particularly when it comes to treating injuries related to contamination. Her role in the narrative is primarily as a medical expert, one who is essential for treating the injuries of the Cleaners and other characters. She is also the grandmother of two other characters, Aesha (also known as Eishia or Eliza) and August, meaning her family is also directly involved in the main events.
