Kakucho Hitto is a major antagonist in the Tenjiku Arc, serving as the right-hand man and most trusted subordinate of Izana Kurokawa. He is recognized as the strongest member of the Tenjiku gang, holding the position of one of its Four Heavenly Kings. His most defining physical features are his buzz-cut black hair and a large, prominent scar that runs from the back of his skull across his left eye, which was rendered blind and pale in color from the childhood accident that caused it. He is most often seen wearing the Tenjiku uniform along with a single long earring in his right ear.

Kakucho's background is marked by profound tragedy and loss. As a young child, he was a close friend of the series protagonist, Takemichi Hanagaki. This friendship ended when Kakucho was involved in a devastating car accident during a family trip in his third year of elementary school. The crash killed both of his parents and left him with the severe scar and blindness that defines his appearance. After the accident, he was placed in an orphanage, a place he would later consider his favorite location because it was where he met Izana Kurokawa. Upon their first encounter, Izana destroyed a makeshift grave Kakucho had built for his parents, declaring that his life was now worthless and ordering him to forget the dead and become his servant. In this moment, Izana promised to build a great kingdom called Tenjiku together, giving the lost and grieving Kakucho a new purpose.

The personality of Kakucho is almost entirely defined by this relationship. Having been left orphaned and alone, he is emotionally closed-off and finds it extremely difficult to trust others. However, he possesses an unwavering and absolute loyalty to Izana, whom he views as his king and master who saved him from a life of emptiness. This devotion is his primary motivation, driving him to fight anyone who stands against Izana, including his own childhood friend Takemichi, whom he states he would kill without mercy. While he often appears serious and business-like, he is not without his own code of honor, as he objects to underhanded tactics like using weapons in a fistfight and expresses disapproval of Izana's plan to murder an innocent person. He is a man who follows his king, but one who internally struggles with the moral consequences of his orders.

In the story, Kakucho acts as the primary enforcer for Tenjiku. He leads the gang's forces in their conflict against the Tokyo Manji Gang. His role is particularly significant because of his pre-existing friendship with Takemichi, which adds a layer of personal conflict to the larger gang war. During the battle at Yokohama Bay, he fights Takemichi directly but also intervenes to stop an ally from using a knife, showing his respect for a fair fight. His key relationships are central to his character arc. With Takemichi, he has a complex dynamic, acknowledging him as his childhood hero while still choosing his loyalty to Izana over their friendship. However, his relationship with Izana is the core of his being. He is not just a subordinate but sees himself as a faithful servant whose only wish is to serve his king and protect him from manipulative figures like Kisaki. This devotion reaches its tragic peak when Izana is shot. Despite Izana having just ordered his execution in a fit of madness, Izana instinctively throws himself in front of Kakucho to take the fatal bullets, revealing that Kakucho was the one person who made him feel less alone.

Kakucho undergoes significant development following Izana's death. Wracked with guilt for having survived when his parents and his king have died, he is left directionless. He later resurfaces as a top executive in other powerful gangs, including Rokuhara Tandai and, in an alternate timeline, Bonten. His journey comes full circle when, after Izana's death, he returns to the orphanage where they met and creates a makeshift grave for his master, finally finding a degree of closure by telling him to rest well.

When it comes to combat, Kakucho is an exceptionally powerful fighter. He is often called the strongest man in Tenjiku, with his raw strength and speed being second only to Izana himself. He demonstrated his prowess by single-handedly defeating several of Toman's vice-captains, including a transformed Angry in his peak "Crying Blue Ogre" state, landing a decisive blow before Angry could even react. His physical power is immense, allowing him to send a large fighter like Benkei flying with a single punch. His notable ability is a full-swing hook punch that he is able to launch from an invisible starting position, a boxing technique he has perfected.
