TV-Series
Description
Caren Hortensia operates as the school nurse at Homurahara Elementary School in Fuyuki City, using this position as cover to monitor the Class Cards phenomenon and observe Illyasviel von Einzbern, Miyu Edelfelt, and Chloe von Einzbern. Her nursing role grants access to children’s suffering, which aligns with her personal inclinations. She exhibits scant professional commitment, frequently dismissing students with minor ailments. Eventually revealed as the Holy Church’s envoy, she manages background operations and oversight for the Class Cards incident.

Caren is the daughter of Kirei Kotomine and Claudia Hortensia. Following her mother’s suicide—an effort to ease Kirei’s anguish—she was entrusted to maternal relatives and adopted their surname. A devout priest raised her without formal schooling, compelling her into church service for eight years until stigmata manifested on her body. This prompted the Church to acknowledge her spiritual worth, leading to her ordination as a priestess with the Cistercian Order at Citeaux Abbey. The Church later dispatched her to Fuyuki to succeed Kirei after his demise and document the Fifth Holy Grail War’s aftermath.

Her physical traits include light gray hair, golden eyes, and pale skin. For school duties, she dons a white nurse coat over a light gray blouse, a mini-skirt, and pantyhose. Her exorcist attire comprises a skirtless dress and stockings, designed for mobility and to entice men. Bandages typically conceal her limbs and neck due to supernatural afflictions, though these are omitted in her school guise.

Caren displays a detached, sadistic demeanor, relishing others' pain—especially children’s. She addresses trivial complaints with cold dismissal, consistent with her Masochistic Spiritualist Constitution: a physiological trait causing her body to mirror end-stage demonic possession symptoms in nearby individuals. This allows her to function as the Church’s "daemon detector." Despite deriving pleasure from suffering, she interprets agony as divine trials, apologizing to God for others' misdeeds rather than blaming them directly. She also manipulates emotional weaknesses in those around her.

Her abilities focus on exorcism. The Masochistic Spiritualist Constitution detects demons by replicating possession symptoms in proximity, a genetically inherited trait crucial for the Church’s early identification efforts. She lacks Magic Circuits but bears stigmata, amplifying her spiritual stature.

Beyond "Prisma Illya 2wei Herz!," Caren appears in other works. In "Fate/hollow ataraxia," she mediates a fabricated Holy Grail War, commanding Lancer and Gilgamesh while forging a nuanced dynamic with Avenger. "Carnival Phantasm" amplifies her manipulative traits comically, underscoring similarities to Kirei. "Fate/Grand Order" depicts her fusion with the divine spirit Amour, granting powers like emotion manipulation via "God Love" chocolates and mental interference resistance. She also surfaces in "Koha-Ace" and "Fate/tiger colosseum," interacting with Kirei, who notes their shared characteristics.
