Confident and outspoken despite her small stature, Chiyoko frequently uses the male pronoun "boku" and exhibits a protective, assertive demeanor. Her central role involves managing Sharaku's dual nature; his exposed third eye transforms him into a malicious genius with supernatural abilities. Aware of this secret, she takes responsibility for applying or reapplying the bandage to suppress his evil side, positioning her as both guardian and strategic partner.

Their relationship varies across media. In the original manga, they are classmates with romantic undertones, and Chiyoko occasionally finds his "evil" state appealing despite its danger. The *Black Jack* TV series shifts their dynamic to a sibling relationship, casting Chiyoko as Sharaku's older sister. Here, she displays a bossy yet caring attitude, urging him to confront bullies and complete homework while remaining fiercely protective.

Other adaptations alter her background. The 2003 *Astro Boy* series reimagines her as Dr. Wato, a magnetic force scientist and Sharaku's mother. She invents a magnetic orb controlling a disassembling robot. After being kidnapped by Skunk Kusai, she is coerced into using her invention for robberies under threats to her son. In *Astro Boy: Omega Factor*, she appears as Professor Ochanomizu's assistant, replacing another character, requiring rescue after a robot abduction.
