Born human in England, Radamathys entered Hades' service after his "malevolent star" awakened. His background reveals noble origins; the Specter Valentine served him even before their underworld allegiance. A pivotal pre-series event saw him battle Leo Ilias in India. Though weakened by illness, Ilias broke one of Radamathys' Surplice horns before sacrificing himself to save his son, Regulus. This encounter instilled lasting respect for Ilias' strength in Radamathys.
During the Sanctuary assault, Radamathys coordinated the Specter invasion of Athena's temple. Defying Pandora's orders, he deployed terrestrial-class Specters to monitor the resurrected Gold Saints, doubting their success in assassinating Athena. When the renegade Gold Saints Cancer Deathmask and Pisces Aphrodite attempted to infiltrate Hades' castle, he intercepted them. Reflecting Deathmask's Underworld Waves, he effortlessly defeated both using his signature technique, Greatest Caution. Following Athena's apparent suicide, he confronted the invading Gold Saints Leo Aiolia, Scorpio Milo, and Aries Mu within Hades' barrier—which suppressed their cosmos to 10%.
