Misato Kiyosato is a young woman with long, straight brown hair, brown eyes, pale skin, and a distinctive mole. She stands 160 cm tall with measurements of 95-60-90 cm (bust-waist-hips).

During medical school, she served as Shuuji's tutor, forming a close friendship where Shuuji developed romantic feelings for her. After graduation, she left medicine and lost contact with Shuuji, marrying Takao Kiyosato, the assistant director of Kiyosato Hospital.

Years later, Shuuji was assigned to the same hospital. Misato expressed happiness at their reunion, unaware Shuuji harbored unresolved trauma and suspicions that Takao caused his father's death during surgery. Seeking revenge, Shuuji committed a non-consensual act against Misato.
