Description
Aiko Katsuragi inherits her family's generational medical practice as the sole physician at Hayashikoshi City Clinic, later known as Rinin Village Clinic. She married her childhood friend Toshio Katsuragi, bearing three children: Shiori, Maki, and Mamoru. Their marriage grew distant, lacking sexual intimacy for over five years and fueling Aiko's frustration. She maintained a sisterly bond with her nurse, Natsuko Hayasaka, who relied on her support.

When Mamoru was caught shoplifting, he avoided police involvement by offering Aiko as a sacrificial "shrine maiden" to the Rinkan Club, a secret society practicing ancient fertility rituals through gang rape. Under duress, Aiko agreed on condition her daughters remained unharmed. During the ritual in a hidden cave, she was gang-raped and mentally shattered, transforming into a submissive priestess who embraced public sexual acts. She performed at the local high school pool, including in front of Maki, and solicited students at the clinic for minimal payment. Shiori intervened by becoming the next shrine maiden to free her. Hospitalized elsewhere, Aiko regained her original personality but experienced amnesia regarding the events, though her body retained traumatic memories.

Despite recovery, Kouji Kashida—a baseball team member recalling her exploitation—later targeted her. After multiple men assaulted her, Toshio rescued her; Aiko confided the experience reawakened latent desires she feared controlling. In the OVA continuity, Suzuko Yamazaki usurped the Rinkan Club leadership, sentencing Aiko, Maki, and Shiori as permanent shrine maidens. This ensured generational enslavement, with future daughters replacing them upon death. All three became pregnant from rituals, their minds broken to perceive the sentence as pleasurable.

In the *Taimanin Asagi Battle Arena (TABA)* timeline (2082), Aiko treated Fuuma Kotaro after he sustained injuries battling demons near Rinin Village. She provided uncompensated medical aid, later requesting a date as payment. Remaining composed amid supernatural threats, she received Kotaro's post-mission contact to confirm the date. An alternate demonic counterpart, "Emilia," appears in the RPGX timeline. Residing in Greenfort Village as a widow with her daughter Arista, she sheltered undercover agent Kuroto (Edwin Black). After bandits attacked her village, Kuroto used his powers to save them; Emilia later directed others amid demonic conflicts.
