U-1146 serves as a neutrophil, a white blood cell patrolling the human body's blood vessels and neutralizing foreign threats like bacteria, viruses, and pathogens. He wears the standard white neutrophil uniform, marked with a cap displaying "白血球" (white blood cell) and his identification number, often stained with cytoplasm during combat. Physically, he possesses pale skin, white hair, and black eyes, with his hairstyle covering his right eye. Neutrophils stand approximately twice the height of red blood cells.

Typically reserved and aloof during patrols, he shifts to aggressive focus in combat. Despite his violent role, he demonstrates kindness and selflessness, prioritizing the protection of other cells without discrimination based on type or rank. He consistently aids a frequently lost red blood cell, AE3803, whom he calls "Cell Boy," interacts respectfully with platelets, and displays empathy, such as listening to an enemy cancer cell's final words. A dry sense of humor surfaces occasionally, like greeting colleagues mid-battle.

Originating in the red bone marrow, he trained as a myelocyte. During this period, he saved a young erythroblast (later AE3803) from a Pseudomonas aeruginosa bacterium. They agreed to reunite as adult cells, though he did not recognize her upon meeting again after maturation. He completed rigorous training to become a fully operational neutrophil, joining the division responsible for immediate pathogen response.

His abilities include phagocytosis (consuming pathogens for analysis or elimination), knife proficiency for close combat, and accurate throwing knives for ranged threats. His receptor functions as a radar detecting nearby antigens, while L-selectin adhesion molecules allow surface adherence during high-speed pursuits. As a wandering cell, he transmigrates through tissues and blood vessel walls for rapid deployment. He coordinates with other neutrophils via chemotaxis transceivers during large-scale threats.

Key relationships involve a protective dynamic with AE3803, whom he rescues repeatedly from pathogens and navigational challenges. He shares camaraderie with fellow neutrophils U-4989, U-2001, U-2626, and U-2048. Conversely, he faces disapproval from Killer T Cell, who opposes inter-cellular friendships. His kindness extends to platelets, whom he treats gently, and good bacteria, whom he tolerates.
