Namie Yagiri, former chief of Yagiri Pharmaceuticals and later Izaya Orihara's secretary, possesses long dark brown hair typically worn down, brown eyes, and a curvaceous physique. Her standard attire is a white lab coat over a dark red mini skirt, black tights, and a green turtleneck sweater.

She exhibits a cold, demanding, and manipulative personality, marked by an intense, obsessive, and incestuous devotion exclusively toward her younger brother Seiji Yagiri. Viewing most human relationships as fundamentally false transactions devoid of genuine emotion, she leverages her intellect and medical expertise—particularly in poisons—as a formidable adversary. However, her arrogance and emotional volatility concerning Seiji frequently precipitate critical miscalculations.

Her childhood was defined by parental neglect following their job loss, forcing her into primary caregiving for Seiji. This role fostered an unhealthy emotional dependence. Raised under their uncle Seitarou Yagiri, she discovered Celty Sturluson's preserved head in his study. Unintentionally revealing it to Seiji ignited his obsessive love for the head, fueling Namie's lifelong resentment toward the object and her drive to control it.

Within the narrative, Namie orchestrated illegal human experimentation through Yagiri Pharmaceuticals, targeting individuals unlikely to be missed. After Seiji stole Celty's head and accidentally injured Mika Harima, Namie covered up the incident. She manipulated Mika into plastic surgery to resemble the head, intending to erase her memories and personality to create a surrogate for Seiji. Mika's escape prompted Namie's pursuit across Ikebukuro, culminating in a confrontation with Mikado Ryuugamine on Sunshine 60 Street. Mikado mobilized the Dollars to trap her; amidst the ensuing chaos involving Celty, Namie fled and sought Izaya Orihara's protection, relinquishing Celty's head to him in exchange for sanctuary and becoming his secretary.

As Izaya's secretary, she retained her manipulative tendencies, occasionally undermining him—such as revealing Mikado's identity as the Dollars' leader to Horada, intensifying gang conflicts. She displayed sardonic amusement at Izaya's misfortunes, like his injury from Simon. Her duties expanded to assisting Izaya in schemes, including impersonating him to manipulate Akane into attacking Shizuo Heiwajima. Despite her antagonism toward Izaya, a reluctant working rapport developed; she occasionally prepared his meals while contemplating poisoning him.

Her obsession with Seiji endured, driving her to stalk his relationship with Mika. She lured Mika to a warehouse, paralyzed her with a drug, and attempted to disfigure her face with acid—justifying an attempted kiss as an "indirect kiss" with Seiji. Seiji intervened after Shinra revealed their location; he kissed Namie to demonstrate the violation of consent. Unaware he blocked the kiss with his finger, she misinterpreted this as mutual affection and retreated elated.

In later arcs, she survived an encounter with her uncle Seijirou and Kasane Kujiragi due to intervention by Shingen Kishitani and allies. She subsequently joined a group coordinated by Walker and Shingen at Celty's apartment, indicating a shift toward collaborative survival amid Ikebukuro's escalating conflicts.
