Physically, he lacks a left eye, typically concealed by his hair, and stands 130 cm tall weighing 30 kg. His primary residence is the GeGeGe Forest, where he inhabits a simple tree house alongside his father, Medama-Oyaji. Medama-Oyaji exists as a reincarnated sentient eyeball after his original body decayed from disease. He serves as Kitarou's guardian and advisor, embodying paternal devotion. Kitarou's mother, Iwako, is acknowledged in lineage but plays no active role due to her demise. Kitarou mediates conflicts between humans and yōkai, striving for peaceful coexistence. He intervenes against malevolent forces disrupting this balance, leveraging his reputation as Japan's strongest yōkai to resolve crises. This role positions him as a de facto leader, with other yōkai seeking his guidance during threats. His abilities include regenerative healing, electrical manipulation, and weaponized techniques like Hair Needles and Finger Guns. He employs artifacts such as the Remote Control Geta (wooden sandals that grant flight), the Yōkai Ocarina for communication, and the protective Chanchanko vest. A notable trait is his tolerance toward Nezumi-Otoko, a rat-human yōkai whose self-serving nature makes him both ally and antagonist. Nezumi-Otoko's betrayals and schemes often necessitate Kitarou's intervention. Kitarou also shares a platonic bond with Neko-Musume, a cat-girl yōkai who occasionally accompanies him during downtime like shopping. In the film *Yōkai Tokkyū! Maboroshi no Kisha*, Kitarou confronts the Western Yōkai Witch after she attacks his ally Ittan-Momen and kidnaps Nezumi-Otoko.
