Totoko Yowai originates from Fujio Akatsuka's *Osomatsu-kun* manga and its adaptations. Her name combines "yowai" (弱い, "weak") and "toto" (トト, a childish term for "fish"), aligning with her frequent depiction as a fishmonger's daughter, though continuity shifts sometimes cast her as the daughter of characters like Dekapan. Her design directly recycles Akko-chan from Akatsuka's earlier work, deemed suitable for her role.

In childhood portrayals (*Osomatsu-kun*), she appears as a neighborhood girl to the Matsuno sextuplets, typically around 10 years old. She is slender and taller than the boys, sporting signature pigtails, a bouffant or bun-like swell atop her head, and a headband (often red or pink) with yellow ribbons. Her hair is black in the manga but deep brown in some anime. She wears dresses or jumper skirts, with the 1988 anime frequently showing a reddish-pink jumper over a light pink blouse. Initially kind-hearted, studious, and friendly, she exhibits low frustration tolerance, especially towards the sextuplets' mischief, retaliating violently with her signature "body blow" punch. Though academically capable, her cooking is terrible. Over time, her manga portrayal shifts: she adopts rougher language, participates in bullying like targeting Iyami, and engages in chaotic acts such as scamming men or skipping restaurant bills. Privately, she resents the pressure to maintain perfection, longing to abandon the "heroine" role. After a paradoxical event transforms her into a monster, she returns to her path upon accepting she need not strive beyond being herself.

As an adult in *Osomatsu-san*, she retains pigtails and a headband but wears adult fashion like a magenta turtleneck, plaid miniskirt, and pink heels. Her core traits of selfishness and attention-seeking persist. She becomes a self-proclaimed "fish idol" to promote her parents' fish shop, performing in a costume featuring a hollowed-out fish mask and octopus-tentacled skirt—a burdensome outfit prone to rotting and foul smells. Her idol career, driven solely by a desire for popularity as revealed in "Totoko's Dream," lacks genuine talent or dedication. Episodes like "Totoko's Big Panic" hint at potential growth when she confronts the reality that her cuteness no longer guarantees privileges in adulthood, but this development is abandoned as she reverts to old behaviors after becoming bored. Her jealousy and need for validation continue, such as competing in an eating contest while internally dismissing rivals or confronting jealous women at a reunion. She remains the object of the sextuplets' affections, showing no consistent preference.

Her relationships center on interactions with the sextuplets, who collectively idolize her and compete for her attention, often leading to rejection or violence from her. She occasionally allies with or manipulates others: scamming Iyami for money by feigning romantic interest, displaying fluctuating dynamics with Chibita (sometimes kind, sometimes antagonistic), and rarely interacting with characters like Hatabo or Dayon. Her family includes a rarely seen mother (who faints at her violent outbursts) and a typically doting father (the fishmonger). An older brother, Fighting Yowai, appears in early manga chapters as a laid-back, stubborn pro-boxer immune to her influence, though phased out of later media.

Her age and role vary across media. In the final *Shonen Sunday* manga story, she appears as an adult secretary for Dekapan. In the *Osomatsu-san* film, she is unemployed but admired for her beauty.
