The Cancer Cell is a recurring antagonist who was born from a copying error in the DNA of a normal cell, which caused him to be a mutation from the very start of his existence. This fundamental error is the root of his identity, as it makes him appear physically different from healthy cells and drives his ability to replicate in an uncontrollable and dangerous manner. His appearance is distinctive and unsettling; his hair is split between light and dark tones, and one of his eyes has a black sclera. Growths resembling bulging veins mark his skin, most notably on his cheeks, and his body is characterized by a constant, grotesque ability to change its shape, size, and even the number of its limbs at will.

The personality of the Cancer Cell is defined by a deep and tragic conflict. He watched his friend be killed by Killer T cells, who cruelly labeled them "goddamn bugs," an experience that fueled his rage and his resolve to destroy the world that rejected him. In one of his most cunning schemes, he successfully deceived Regulatory T Cell into protecting him, exploiting her function of recognizing and defending native body cells. His most significant relationship is with the neutrophil U-1146. The Cancer Cell develops a strange fondness for this white blood cell because U-1146 once protected him while he was in disguise, showing him a kind of kindness and seeing him as a living being rather than just a threat to be eliminated. He ultimately expresses a preference to die by U-1146's hands rather than those of any other immune cell, a testament to the unique and tragic bond between them.

Throughout the story, the Cancer Cell demonstrates significant development, primarily through his defeats and subsequent returns. He was killed by the immune cells, but he ominously warned that their battle was not over. True to his word, he returned in a later story arc, resurrected after his genetic data was leaked, allowing him to reappear with his memories intact. Upon his return, he was empowered by the toxins present in an unhealthy intestine, making him even more formidable than before. This ability to recur is one of his most terrifying traits, suggesting that the threat he poses is not easily vanquished.

To achieve his goals, the Cancer Cell possesses a terrifying array of abilities. He is a formidable hand-to-hand combatant, capable of taking on multiple immune cells at once. His primary power is his shapeshifting mutation, which allows him to dramatically alter his form in the middle of a fight, sprouting multiple limbs or giant fists to overpower his enemies. He can also regenerate from non-fatal injuries, making him incredibly difficult to put down. When he is ready to spread cancer throughout the body in a process called metastasis, he grows a pair of large feathered wings to travel quickly. Furthermore, his power seems to grow stronger in the presence of bacterial toxins, allowing him to draw strength from a diseased environment. His ability to disguise himself as a normal cell is perhaps his most insidious power, allowing him to move freely and manipulate other cells to further his destructive plans.
