Dr. Kureha is a highly skilled and remarkably long-lived physician from Drum Island, a winter island in the Grand Line. Known to the local populace as a witch, she is an eccentric and formidable woman of advanced age, being one hundred and forty-one years old. Her background before arriving on Drum Island is largely shrouded in mystery, though she possesses esoteric knowledge that few others have, such as the true name of the Pirate King, Gol D. Roger, and an awareness of the mysterious Will of D.

Kureha's personality is as striking as her medical abilities. She is often called a witch due to her unorthodox and aggressive treatment methods, her habit of charging exorbitant fees that often amount to half of a patient's possessions, and her reclusive nature. She does not suffer fools gladly and is quick to threaten or physically assault anyone who disobeys her medical advice or, most offensively, calls her old or a hag. She takes immense pride in her youthfulness and vitality, frequently asking others if they want to know the secret to her longevity, even when no one has asked. Despite her gruff and mercenary exterior, she has a deep-seated and genuine love for her adopted son and former student, Tony Tony Chopper. She possesses a strong sense of responsibility as a doctor, firmly believing that a patient can only leave her care in one of two conditions: fully recovered or deceased. She enjoys drinking plum sake and is seldom seen without a bottle nearby.

Her motivations are rooted in a profound commitment to the art of healing. She was one of only two doctors on Drum Island who refused to bow to the tyrannical King Wapol's decree that all physicians must work exclusively for him, continuing to treat the sick in secret. Her primary role in the story is as the mentor and surrogate mother to Chopper, taking him in after the death of his father figure, the quack doctor Hiriluk. She trained Chopper for several years, imparting to him her vast medical knowledge and helping him develop his skills to become a true doctor capable of curing any disease. She is responsible for Chopper's ability to create the Rumble Ball, a drug that alters the wavelengths of his Devil Fruit transformation. Later, she plays the crucial role of pushing Chopper to leave the island and join the Straw Hat Pirates, recognizing that he needs to see the world to fully grow.

Kureha's key relationships are central to her character. Her bond with Tony Tony Chopper is one of tough love. She was initially furious with him for unknowingly poisoning Hiriluk but subsequently honored the dying man's request to train the young reindeer. She treats Chopper as her son, expressing her affection through stern lectures and acts of hidden kindness, such as preparing his medical bag for his journey and creating a spectacular farewell of artificial cherry blossom snow. Her relationship with the late Dr. Hiriluk was a complex friendship between the only two free doctors on the island. Throughout the series, Kureha shows little personal development, as she is already a fully realized and century-old master of her craft. Her significance comes from her impact on Chopper and the world around her. She remains a constant figure on Drum Island, now renamed the Sakura Kingdom, and continues her life as the head of a vast medical network, the Isshi-100. She later accompanies King Dalton to the Levely, the world's most important political gathering, demonstrating her continued vigor and importance even in the later stages of the story.

Notable abilities define Dr. Kureha as one of the finest medical minds in the world. Her expertise is so profound that she was able to cure Nami of a disease caused by an extinct, venomous tick, a feat that would have been impossible for an ordinary doctor. She is also one of the few medical professionals globally authorized to use the highly restricted drug, NHC10. Beyond her medical prowess, Kureha possesses astonishing physical capabilities for a woman of her age. She has demonstrated superhuman strength, easily smashing through stone walls, and immense speed and agility, allowing her to chase and throw a variety of weapons with deadly accuracy. She has a technique called Doctor Stop, a brutal form of physical therapy where she forcibly prevents patients from doing further harm to themselves, such as stomping on an injured Sanji to keep his damaged spine from breaking. Her incredible resilience to cold and her overall physical vitality are a testament to her philosophy that a doctor must also practice what they preach regarding health and happiness.
