Dr. Tim Marcoh is a former State Alchemist who held the title of the Crystal Alchemist. He is an elderly man, appearing short in stature with broad shoulders, and his aged face is marked by prominent wrinkles and a large jaw. His hair is dark, though grey at the sides, which demonstrates his advanced years. He is often initially seen wearing a laboratory coat, a remnant of his days as a military researcher.

Marcoh’s past is defined by a profound tragedy for which he holds himself personally responsible. During the Ishvalan Civil War, he was ordered by the military to lead a top-secret project devoted to the creation of the Philosopher's Stone. Through horrific means that involved the sacrifice of innocent Ishvalan lives, he succeeded in producing imperfect copies of the Stone. These stones were then given to State Alchemists on the front lines, amplifying their powers to a devastating degree. The result was the swift and brutal end of the Ishvalan rebellion, but at the cost of an enormous number of civilian casualties, an act that horrified him.

Haunted by immense guilt and self-loathing for his role in the massacre, Marcoh deserted the military. He fled to a small, remote town where he took on the alias "Dr. Mauro" and attempted to atone for his sins by working as a physician, using his alchemical knowledge and a remaining sample of the imperfect Stone to heal the sick. He is, at his core, a kind and gentle man who deeply cares for others, but he is also profoundly passive and tormented. Having been forced to comply with the military's inhumane orders, he developed a weak and passive personality, unable to refuse when pressured by others. When first confronted by Edward Elric and Major Armstrong, he mistakenly believes they have been sent to retrieve him and reacts with fear and desperation, even attempting to shoot Armstrong. His self-loathing is so deep that, upon meeting Scar, an Ishvalan whose people he helped destroy, he begs to be killed, referring to the avenger as a god come to deliver punishment. He holds no grudge against Scar for later horrifically disfiguring his face as a disguise, accepting it as the face he deserves.

In the narrative, Marcoh's role is initially that of a source of critical information. He directs the Elric brothers to the location of his hidden research notes, which contain vital secrets about the nature of the Philosopher's Stone. His past catches up with him, and he is captured by the Homunculi, who wish to exploit his knowledge for their own plans. During his imprisonment, he is constantly tormented by Envy, who holds the safety of the villagers from his new home as a hostage for his cooperation. This period of powerlessness proves to be a turning point; his rescue by Scar provides an unexpected opportunity for resistance. Despite expecting death, Marcoh finds a new purpose in helping Scar and his allies dismantle the Homunculi's conspiracy. In a pivotal act of defiance, he uses his unique knowledge to attack Envy, activating a hidden transmutation circle that critically damages the Homunculus' core Stone, proving that he is capable of fighting back using his intellect. His development sees him transform from a passive, guilt-ridden fugitive into an active, if reluctant, ally who uses his shameful past to redeem himself.

Marcoh's key relationships are central to his character arc. His encounter with Edward Elric reopens a door he thought he had closed, but he entrusts the boy with his dangerous knowledge. His relationship with Scar is deeply complex; the Ishvalan represents both the punishment Marcoh feels he deserves and the means to take meaningful action against those who orchestrated the tragedy. Finally, while under the care of Dr. Knox, he approaches Roy Mustang with an offer: to use his knowledge and a Philosopher's Stone to restore the Colonel's eyesight, on the condition that Mustang dedicates his new position to the restoration of Ishval and the improvement of conditions for its people. Marcoh also expresses his intention to stay in Ishval and work as a doctor, a final step in his journey toward taking personal responsibility for his nation's crimes.

In terms of notable abilities, Dr. Marcoh is recognized as one of the oldest and most knowledgeable alchemists in the series. His expertise in medical alchemy is unparalleled, a skill he honed during his time as a researcher. His most infamous and significant ability, however, is his intimate knowledge of the Philosopher's Stone. Not only did he lead the project to create it, but through his countless experiments, he also learned how to destroy it. This specialized skill makes him one of the few individuals capable of fighting a Homunculus on even terms, as he can target the core Stone that sustains their immortality. While he is elderly and not suited for direct combat, his strategic and supportive abilities are invaluable. He is capable of deciphering complex alchemical codes and using a Philosopher's Stone to perform powerful healing transmutations, mending otherwise fatal wounds and permanent disabilities.
