Description
Dr. Heihachiro Ikuma is a central figure in the prequel film Ring 0: Birthday, where his actions directly shape the tragic fate of Sadako Yamamura. He is introduced as a doctor and the lover of Shizuko Yamamura, Sadako's mother, making him Sadako's stepfather and presumed father figure, though the films strongly hint that her biological origins are something far more otherworldly. His professional background as a doctor and his fascination with psychic phenomena first drew him to Shizuko, leading to an affair that resulted in Shizuko's pregnancy.

In Ring 0: Birthday, Ikuma's personality is defined by a profound and tragic conflict between his sense of responsibility and his overwhelming fear. He is portrayed as a man who attempts to control a situation that is spiraling into supernatural chaos. After Shizuko's death, he takes responsibility for Sadako, but he quickly discovers that she has split into two distinct beings. One is a timid, benign, and gentle girl, while the other is a malevolent and powerful psychic entity. His solution to this impossible situation reveals a calculated, clinical, and desperate side of his nature. He allows the gentle Sadako to live a relatively normal life, even letting her grow up and attend school, believing she is harmless and perhaps worth saving. In stark contrast, he locks the evil Sadako away in the attic of his home, feeding her growth-inhibiting hormones to keep her trapped in the form of a small child, a method that reflects his medical knowledge but also his moral compromise.

Ikuma's primary motivation throughout the prequel is containment and the prevention of tragedy. He is driven by a wish to protect others from the terrifying power of the evil Sadako, and perhaps to protect the gentle Sadako from her own darker half. He is not a villain in the traditional sense, but a man pushed to horrifying extremes by forces he cannot fully comprehend or control. However, his careful but fragile system fails when the two halves of Sadako are finally reunited. Faced with the fully merged, incredibly powerful, and grief-stricken Sadako, who has just killed several people, Ikuma makes the ultimate, desperate decision to end her life. He tricks her into drinking poison before initiating a violent struggle, during which he strikes her with an axe and disposes of her body by throwing her down a deep well. This brutal act is presented not as simple cruelty, but as a tragic climax of horror and despair. Immediately after throwing her into the well, Ikuma breaks down in tears, showing deep remorse and sorrow for the act he felt was necessary.

Key relationships define his role in the story. His relationship with Shizuko Yamamura begins as a professional fascination that turns into a romantic affair. After her death, his entire world becomes defined by her daughter, Sadako. He acts as a guardian and jailer, attempting to separate and control her dual natures. In the later films of the original Ring series, specifically the 1998 film Ring, an older Ikuma is confronted by the characters Reiko Asakawa and Ryuji Takayama, where he confesses to his crime and reluctantly reveals the full, tragic history of Sadako.

Dr. Heihachiro Ikuma does not possess supernatural abilities himself. His notable skills are entirely human, grounded in his profession as a doctor. He uses his medical knowledge to administer drugs and hormones to Sadako, attempting to suppress the growth and power of her evil half. His most significant ability, however, is his willingness to take absolute, violent action in what he perceives as the greater good, a trait that ultimately makes him both a tragic protector and the man responsible for Sadako's descent into the well and her transformation into the vengeful spirit of the curse.
