The opening scene of "Grimm Variations - Cinderella" Episode 1 shows Jacob and Wilhelm Grimm rewriting the fairy tale of Cinderella. They are accompanied by their sister Charlotte, who expresses concern for Cinderella but is reassured by her brothers. The story then shifts to present-day Japan.

In this version, Viscount Otawara proposes to the geisha Tsuruko, who hesitates out of concern for her daughter Kiyoko. The Viscount reveals that Kiyoko has been wishing for a new family. Tsuruko and her daughters Makiko and Sawako arrive at the Otawara estate, where they are greeted by Kiyoko. The daughters quickly display their lack of manners, and the household staff finds them insolent.

Kiyoko attempts to befriend her new stepsisters by mimicking their behavior and showering them with luxury gifts, which leads to conflicts between Makiko and Sawako. A maid is falsely accused by Makiko of stealing a hairpin and is fired by Tsuruko after a confrontation.

After an incident where Kiyoko fakes a fall, her father becomes furious with Makiko and Sawako but is appeased by Kiyoko's pleas. Kiyoko then begins giving away her valuables to her stepsisters and behaving submissively. Due to their behavior, the sisters grow unpopular in the household.

Viscount Otawara decides to send Makiko and Sawako abroad due to their misconduct. Tsuruko remains unconvinced that Kiyoko is deceiving them until a deliberate interaction occurs. Later, the Viscount dies in a car accident, leading to further shifts in the household dynamics. Kiyoko privately celebrates his death.

Years later, Makiko and Sawako have taken control of the household, while Kiyoko subtly manipulates the family dynamics. An invitation to a count's ball offers Makiko and Sawako a chance to escape Kiyoko's influence, but Kiyoko undermines their plans, leading to their social downfall at the event.

Kiyoko's manipulations continue as she poses as a caring sister while secretly harming those around her. The episode ends with the sisters being disowned and freed from Kiyoko's schemes, but Kiyoko is already planning her next move, which involves the count's son.
