The episode begins with a flashback to Dragon Boy's origin: As a young man, he is recruited by Rock after proving himself in a brutal fight. Rock explains that he wants to shape him into a "god" like Zero, and Dragon Boy agrees in order to eliminate "scum" from society.

In the present, Rock and Dragon Boy discuss the current hero rankings. Due to the increased popularity of Ghostblade, Loli, and Little Johnny after the ruins incident, they decide to focus instead on the long-time top 10 hero Smile. Rock hands Dragon Boy a mysterious small box as the "deadliest weapon" for this mission.

In parallel, Nice is advised by his idol Smile during a photoshoot that a real smile must come from within. However, his moment of joy is abruptly disrupted when Shand reminds him that he is merely a puppet. Shand orders Nice to replace Smile and break into the top 10 in order to take revenge for his son's death on E-Soul and Rock.

Meanwhile, Smile visits the current hero X in his apartment. Their conversation reveals that they are close friends who are both weary of being heroes. Smile confesses that his eternal smile is a facade and that he suffers from alienation from his family.

After leaving X's apartment, Smile is confronted by Dragon Boy. The latter provokes him with his daughter's necklace, claiming to have killed her. A brutal fight breaks out, pitting Dragon Boy's ability to grow stronger from hits taken against Smile's experience and raw power. When Smile briefly gains the upper hand, Dragon Boy summons Nice, who has been hiding in the shadows with a vial of Fear matter.
