Originally from Takarazaki City, she transfers to Kamihama City to investigate mysterious dreams concerning her younger sister, Ui Tamaki, who suffered from an incurable congenital disease. Iroha contracted with Kyubey, wishing desperately, "Please! Please cure Ui! Make my precious little sister healthy! If you can do that, I'll do anything for you!" This wish tragically erased Ui from existence, wiping all memories of her, including Iroha's recollection of making the wish itself.

Together, they opposed the Magius—an organization seeking to "save" magical girls by exploiting Uwasa (rumor-born entities) and creating the doppel system. The Magius' leaders, Touka Satomi and Nemu Hiiragi, were Ui's former friends but possessed no memory of her.
Iroha discovered Ui was fused with the Magius' creation, Embryo Eve. After convincing Touka and Nemu of Ui's existence, Iroha rescued her sister during a climactic battle against Eve and the witch Walpurgisnacht.
