Description
Dr Nako is the resident physician of Cocoyasi Village, a small community in the East Blue that falls under the tyrannical control of the fish-man pirate Arlong. He is an elderly man with a wrinkled face, grey hair, and a distinctive long goatee. His typical attire consists of a doctor's coat, a bandana marked with a red medical cross, and a pair of sunglasses, giving him a stern and focused appearance.
His personality is defined by a blend of gruff pragmatism and profound, unspoken care for those around him. Dr Nako possesses a very matter-of-fact and logical disposition, often forgoing sentimentality to make difficult, realistic decisions. This can make him appear harsh or grumpy, such as when he scolds the young Nami or expresses disbelief that the pirate Monkey D. Luffy has no doctor on his crew to treat severe wounds. However, this stern exterior masks a deeply compassionate and brave individual. He has demonstrated remarkable courage, joining the other villagers in a seemingly suicidal charge against the Arlong Pirates, fully aware that they stood no chance against the fish-men.
Dr Nako has known Nami and her adoptive sister Nojiko since they were infants, having helped care for them when they were first brought to the village by their mother, Bell-mère. Following the murder of Bell-mère and Arlong's occupation of Cocoyasi, Dr Nako, alongside his close friend and the village sheriff, Genzo, became a de facto leader for the community. In his role as an elder, he has often been tasked with confronting the painful realities of their situation. Recognizing the danger posed by Arlong, Dr Nako was one of the adults who attempted to force the young Nami and Nojiko to flee the island for their own safety, a decision that wounded him to make but one he believed was necessary.
His primary function as a physician is evident throughout the Arlong Park conflict. After the Straw Hat Pirates ultimately defeat Arlong, Dr Nako is responsible for treating the serious injuries of Roronoa Zoro, as well as Luffy, Yosaku, and Johnny. It is during this time, while tending to wounds that would be life-threatening for an ordinary person, that he grumbles in disbelief about the crew lacking a dedicated doctor, inadvertently giving Luffy the idea to recruit one in the next arc. Beyond his medical expertise, Dr Nako possesses a unique secondary skill in tattooing. He performs a deeply symbolic act by removing the mark of the Arlong Pirates from Nami's shoulder. To cover the remaining scar tissue, he applies a new tattoo based on a design Nami provided, which features a pinwheel (representing Genzo) and a tangerine (representing her mother, Bell-mère), transforming a brand of enslavement into a tribute to her family.
