Tsukiko Izumisawa, a high school student with black hair, funds herself through covert photography sessions, capturing and selling images of popular male classmates. Her clandestine business draws the ire of transfer student Tomie, who confronts Tsukiko over the unauthorized pictures. Tsukiko subsequently avoids Tomie. However, Tsukiko's crush, Yamazaki, specifically requests photographs of Tomie. Complying, Tsukiko takes the pictures but is caught and reprimanded by a teacher during the act. Developing the film reveals a shocking anomaly: Tomie appears with two heads. Seeking retaliation, Tsukiko distributes copies throughout the school. This act angers Yamazaki, who then attempts to murder her. Tsukiko narrowly escapes the attack. Later that day, Tomie visits Tsukiko's home, offering an apology Tsukiko rejects. Provoked by the rejection, Tomie manifests what Tsukiko perceives as her "true form," revealing the second head. Displeased by this transformation, Tomie instructs her associates to remove the extra head. Instead, they decapitate her entirely in Tsukiko's room. Witnessing the murder, Tsukiko is restrained by Tomie's associates to prevent her reporting the crime. They take the severed head to burn it, but Tomie begins regenerating a new head and escapes. The entire traumatic sequence concludes ambiguously, leaving open the possibility that Tsukiko experienced these events as a nightmare.
