Left Hand, who appears as a sentient, parasitic face residing in D's left palm, is a symbiotic entity that has existed alongside the dhampir hunter for an extended period, perhaps since his birth. Its origins are tied to D's unique nature, serving as a constant companion and resource. The entity's personality is sharply defined by pragmatism, sarcasm, and a dry, often morbid sense of humor. It frequently serves as D's internal voice of reason and self-preservation, voicing the practical thoughts and survival instincts that D himself keeps buried beneath his stoic exterior. Its primary motivation is the preservation of its own life, which is directly linked to D's survival, driving it to constantly seek energy and advise caution. In the story's unfolding, Left Hand fulfills several critical roles. It acts as a wellspring of esoteric knowledge, providing D with information about vampires, their powers, and the history of the world. It also functions as a potent healer, capable of mending D's injuries and even reviving him from the brink of death by expending its own energy. Key relationships are almost entirely defined by its bond with D; there is a mutual dependence, with Left Hand providing vital support while D provides it with a host body and a means of experiencing the world. The entity shows little to no personal development over the course of the hunt, remaining a static force of pragmatic survivalism.
