Description
Doctor Kureha is an exceptionally long-lived physician originally from Drum Island, which later becomes the Sakura Kingdom. She is known by the epithet "Witch" due to her eccentric methods and formidable personality. At the time of her first appearance, she is 139 years old, and after the two-year time skip, she is 141, making her one of the oldest human characters in the world, a fact she takes great pride in. Despite her advanced age, she maintains a physique and level of energy comparable to a woman in her prime, often wearing clothing that exposes her midriff even in the island's freezing climate. She is exceptionally tall, standing 188 centimeters, with long, light gray hair and a preference for purple blazers and accessories like sunglasses and a navel ring.

Her personality is blunt, forceful, and fiercely independent. She despises being addressed as old or a hag and will violently retaliate against anyone who does so, often with kicks or thrown objects. She frequently begins conversations with the rhetorical question, "Want to know the secret of my youth?" though no one ever seems to take her up on the offer. She is also known for her heavy drinking, always keeping a bottle of plum sake close at hand. Beneath this rough exterior, however, lies a deeply principled and caring individual. Her philosophy as a doctor is absolute: a patient will only disappear from her sight if they are either cured or dead, and she never abandons a case. To ensure this, she is notorious for charging exorbitant fees, often taking half of a patient's assets, which ensures that even the poor can pay with what little they have while the rich are held to the same standard.

Doctor Kureha's motivations are rooted in a sense of professional pride and a personal mission to preserve and advance medicine in her homeland. During the reign of the tyrant King Wapol, who outlawed all physicians except his personal staff, Kureha remained as one of the only two doctors in the country who continued to treat the people, the other being the quack doctor Hiriluk. She respected Hiriluk's spirit despite considering his methods foolish, and after his death, she agreed to his final request: to train his adopted son, a blue-nosed reindeer named Tony Tony Chopper, in the art of real medicine. She saw potential in the outcast creature and took him on as her apprentice, becoming a surrogate mother to him.

Her role in the larger story is most pivotal as Chopper's mentor. For six years, she taught him everything she knew, from surgery to pharmacology, shaping him into the capable doctor of the Straw Hat Pirates. When the Straw Hats arrive on Drum Island seeking a doctor for Nami, Kureha cures Nami's rare illness and ultimately decides that Chopper must leave with them to grow. She creates an elaborate, violent charade of driving him away with thrown weapons and harsh words, secretly giving him a bag of supplies and using Hiriluk's life's work to create a spectacular firework display of pink cherry blossoms in the snow, finally calling him her "foolish son" as she bids him farewell. After the timeskip, her role becomes more political. She appears at the World Council alongside King Dalton, now the head of the country's expanded medical corps, the Isshi-100, demonstrating that she has trained a new generation of doctors to secure the kingdom's future.

Key relationships define her character. Her bond with Chopper is the most significant; while she rarely shows open affection, she considers him her son and takes immense pride in his growth as a doctor. Her relationship with Hiriluk was one of mutual respect mixed with exasperation. She thought him a fool for chasing impossible dreams, but she ultimately carried his will forward by making the cherry blossoms bloom and ensuring his legacy lived on through Chopper. Her interactions with the Straw Hats, particularly Luffy and Sanji, are brief but memorable, as she shows a grudging respect for Luffy's determination and is impressed by his will.

Kureha's development is subtle but profound. She transitions from a solitary witch living in isolation to a leader and nation-builder, taking on the responsibility of not just one pupil but an entire country's medical infrastructure. She becomes a representative of the Sakura Kingdom on the world stage, showing that she has accepted a public role she never sought before.

Her abilities are considerable. She is an unparalleled physician, possessing knowledge of medicines and diseases thought to be extinct, and is one of the few doctors authorized to use the highly restricted drug NHC10. She is extremely adept at non-invasive diagnosis, able to determine a fever's exact temperature by touch alone. She also demonstrates significant physical prowess for her age; she can kick through solid brick walls, possesses superhuman speed and reflexes, and has incredible accuracy when throwing a seemingly endless supply of knives and other projectiles. She is also highly resilient to cold, a necessary trait for living on Drum Island.
