Description
Sanpei Kawahara is a young boy whose distinct physical resemblance to a kappa, a mythical water creature, makes him a target for bullying. He lives in a rural mountain town with his grandfather, while his mother supports his future education by working at a Tokyo pachinko parlor. His father, Jūrō Kawahara, vanished after isolating himself to protect the secrecy of a dwarf society he studied.

Sanpei's life shifts dramatically when kappa, mistaking him for one of their kind, abduct him to Kappa Country. Though humans entering their realm face execution, Sanpei's appearance saves him. He returns to the human world on the condition he cares for Kanpei (also called Kanko or Gātarō), the Kappa Elder's son studying abroad among humans. This begins Sanpei's enduring link to the supernatural.

Tragedy follows Sanpei. A Shinigami takes his grandfather despite his efforts. Later, he reunites with his reclusive father, only for the Shinigami to return and claim his father's life too. These losses deepen Sanpei's abandonment and poverty, though his father entrusts him with protecting the dwarfs before dying.

Sanpei juggles multiple burdens: safeguarding the dwarfs, managing Kanpei's often unnoticed antics in the human world, and confronting entities like a mischievous tanuki and the persistent Shinigami. In some narratives, the Kappa Elder enchants Sanpei, granting him kappa-like abilities such as firing water shurikens to battle threats like the tree demon Mizu-Oni.

His adventures extend into searches for his mother, taken to the yōkai world. Across adaptations, he partners with Rokubei, a guardian kappa, and Kanpei to rescue her, encountering diverse yōkai.

Certain storylines reveal Sanpei's explicit hybrid heritage—one ancestor was a kappa. This lineage blurs human and yōkai boundaries, sometimes allowing him to switch places with Kanpei undetected. He endures repeated hardships, culminating in some versions with his death and subsequent ghostly existence struggling to accept his fate.

- Rental manga (1961–1962) focuses on Kanpei's study abroad.
- The Shōnen Sunday version (1968–1969) adds subplots, including a quest for magical artifacts hidden by a kappa king.
