Akemi Miyano is the older sister of Shiho Miyano (later Ai Haibara). Both sisters belonged to the Black Organization, with Akemi holding a lower rank than Shiho, permitting her a semblance of normal life including school and college, albeit under surveillance. Orphaned young when their parents, Atsushi and Elena Miyano, died in an accident, the sisters remained under the organization's control. Akemi's driving purpose was securing her sister's freedom. Approximately five years before the main storyline, Akemi met Shuichi Akai during his infiltration of the organization under the alias Dai Moroboshi. Though Akai initially pursued the relationship to access Shiho, genuine affection developed between them. Akemi deduced his FBI affiliation but kept this secret. After Akai's cover was blown and he was expelled, Akemi expressed her wish to leave with him but refused to abandon Shiho. Deeming her a liability due to her connection to Akai, the organization orchestrated her elimination via a bank heist. The organization deceived Akemi into robbing a bank for one billion yen with false promises of freedom for herself and Shiho—a plan engineered for her failure. She recruited two partners: getaway driver Kenzo Hirota and an unnamed accomplice. The night before the heist, she texted Akai asking if he would date her if she escaped. Following the robbery, Kenzo betrayed them and fled with the money. To find him, Akemi adopted the alias Masami Hirota, posing as Kenzo's daughter from Yamagata, and enlisted Kogoro Mouri's help. During this investigation, she met Shiho at a restaurant, describing Conan Edogawa's unusual maturity and proximity to the Kudo residence, urging Shiho to abandon the APTX 4869 project. After retrieving the money, Akemi poisoned her remaining accomplice using pills Gin provided, unaware they contained cyanide. She delivered the money to Gin and Vodka at a shipping yard, demanding Shiho's release. Gin reneged, citing Shiho's indispensability, and shot Akemi when she refused to reveal the money's location. As she lay dying, Conan revealed his true identity as Shinichi Kudo; she warned him of the organization's danger and disclosed the money's whereabouts. Her death was ruled a suicide due to her fingerprints on the weapon. Akemi's personality blended outward cheerfulness, nurturing warmth, and resilience, masking private emotional pain. As a child, she exhibited playful mischief, like hiding tools to make adults rest. She maintained a close bond with Shiho, serving as her emotional anchor. Her cleverness manifested in orchestrating the heist and creating complex codes. Physically, she resembled Ran Mouri, with long dark hair and a medium build, a likeness noted by both Shiho and Akai. Unresolved elements surround Akemi. She recorded messages for Shiho before her death, suggesting foreknowledge. Theories speculate on her potential survival, citing inconsistencies like Gin shooting her abdomen instead of the head and her repositioning after being shot. Gosho Aoyama's comment about Conan's "mistake" in revealing his identity further fueled speculation. Her detailed knowledge of Conan's behavior before their onscreen meeting also implies possible offscreen surveillance or investigation.
