It takes the form of a spectral female figure draped in tattered robes, frequently seen floating above combat zones or near soldiers marked for death. This entity patrols active war zones, collecting the souls of fallen warriors and embodying the pervasive fear and psychological trauma endured by frontline combatants.

Its presence becomes an arc word among Federation soldiers, linking both the terror of encountering Zeon's mobile suits and the inevitability of their own demise. The Grim Reaper often manifests visibly to individuals under extreme stress or facing imminent death, such as Lieutenant Ben Barberry, whom it taunted as the sole survivor of his unit before his final battle with a Zaku.

The entity operates as a narrative device connecting disparate episodes, observing and influencing human actions fueled by vengeance or despair. In a significant intervention, it revealed to the mortally wounded Lieutenant Arleen Nazon that her suicidal act to kill her traitorous lover stemmed from a tragic misconception – her lover was actually an undercover Federation agent.
