Tomoaki Araide serves as a physician and occasional basketball coach at Teitan High School, attended by Ran Mouri and Shinichi Kudo. He is approximately 25 years old. His father, Yoshiteru Araide, was Kogoro Mouri's physician until his murder during a dinner with the Mouri family and Conan Edogawa; Yoshiteru was electrocuted in his bathtub. The murderer was Tomoaki's stepmother, Yoko Araide, who sought revenge for the death of Tomoaki's mother, Chiaki Araide. Chiaki died in an apparent drunk-driving incident four years prior, despite disliking alcohol, and Yoko believed Yoshiteru's infidelity drove Chiaki to despair. During this case, Conan mistakenly interpreted Ran's attention towards Tomoaki as romantic interest, though she was actually noting the pattern of his sweater to replicate it for Shinichi.
Following his father's death, Tomoaki inherited the family clinic. His identity was later usurped by Vermouth of the Black Organization, who impersonated him to locate Ai Haibara. FBI Agent Jodie Starling intervened, rescuing Tomoaki and his family when Vermouth moved to eliminate them to preserve her cover. After this incident, Tomoaki returned to Japan and resumed his medical practice in Beika. Jodie informed him of Conan's involvement with the FBI, though Tomoaki remained unaware of Conan's true identity or the full scope of the Black Organization's activities.
Tomoaki reappeared at Teitan High during a ghost story case, assisting students and displaying limited knowledge of Vermouth's impersonation. He expressed surprise upon learning Vermouth's impersonation was well-liked, prompting Jodie to clarify that Vermouth merely copied his personality. In another case, he cared for stray kittens at his hospital after encountering the Detective Boys during a hit-and-run investigation.
The non-canon OVA *The Stranger from Ten Years Later* depicted a fever-induced hallucination experienced by Conan. In this scenario, a future Tomoaki developed romantic feelings for Ran and proposed, but she rejected him, stating she would wait another ten years for Shinichi's return. This storyline is explicitly framed as a dream resulting from Conan's high fever and experimental antidote side effects.
Tomoaki Araide is consistently portrayed as kind, trusting, and dedicated to his profession. He believes in people's inherent goodness, sometimes to a fault, and is quick to assist others. He maintains professional relationships with characters like Ran and Conan, with no canonical romantic developments between him and Ran beyond the OVA's dream sequence.
