His combat relies on the Scarlet Needle technique, targeting fourteen points on an opponent's body mirroring the Scorpio constellation's stars. Each strike inflicts escalating pain, sensory deprivation, and bleeding, with the final blow, Antares, usually proving fatal. Despite this brutal method, Milo consistently offers opponents the chance to surrender before enduring the full sequence. He also wields Restriction, a cosmos-based power to immobilize foes.
In the Sanctuary arc, Milo overpowered Bronze Saints Seiya and Shiryu. His pivotal confrontation was with Cygnus Hyoga. After Hyoga resisted Restriction due to icy defenses, Milo delivered fifteen Scarlet Needles. Near defeat, Hyoga awakened his seventh sense, launching an Aurora Thunder Attack simultaneously as Milo struck with Antares. Witnessing Hyoga's resolve to fight on despite his injuries, Milo conceded victory. He realized Hyoga's attack had frozen the critical points on his Gold Cloth, points that would have been fatal without its protection. Milo then stopped Hyoga's bleeding and later used his own blood to repair the damaged Cygnus Cloth as atonement, revealing his capacity for mercy and respect for unwavering loyalty to Athena.
During the Hades Chapter, Milo played a key role testing Gemini Kanon's loyalty after Kanon arrived at the Sanctuary pledging aid to Athena. Skeptical due to Kanon's past treachery as the Sea Dragon General, Milo subjected him to the Scarlet Needles, ignoring Athena's initial pleas for forgiveness. Kanon endured without retreating. After delivering what seemed to be Antares, Kanon collapsed but later rose, revealing Milo had faked the final strike to halt Kanon's internal bleeding instead of killing him.
