Totoko Yowai, a central female character from the *Osomatsu-kun* franchise, derives her name from the Japanese word for "weak" (yowai) and a childish term for fish ("toto"), reflecting her common role as a fishmonger's daughter. Her character design repurposes Fujio Akatsuka's earlier creation, Akko-chan from *Akko-chan's Got a Secret!*, specifically reusing a "gentle face" design.

As a child in the original manga, Totoko appears as a kind-hearted, studious, and friendly girl residing near the Matsuno sextuplets. Her family background and age lack consistent continuity. She is typically depicted as slender and taller than the sextuplets, wearing pigtails with a headband and ribbons, and dressed in dresses or jumper skirts. Despite her generally gentle demeanor, she exhibits a short temper and violent tendencies when frustrated, especially towards the sextuplets' antics, employing her signature "body blow" attack. She excels academically but is notably terrible at cooking. The sextuplets idolize her and compete for her attention, though she often punishes them for causing trouble. She shares intermittent friendly or antagonistic interactions with characters like Chibita and Iyami.

The 1988 anime adaptation significantly alters her personality, portraying her as vain, selfish, and obsessed with money, fame, and attention. This version amplifies her two-faced nature, showing her scamming others, skipping bills, and even comically attempting murder when ignored or spurned. She occasionally engages in chaotic acts with the sextuplets, such as bullying Iyami. Some episodes retain her more innocent or heroic traits.

As a teenager in the *Osomatsu-san* movie, the 18-year-old Totoko adopts a mature image with loose hair and a high school uniform, presenting herself as a humble, intelligent school idol. Internally, she resents the pressure to maintain perfection and secretly desires to abandon her "heroine" role to live freely. A pivotal event transforms her into a monster, resolved after accepting she can prioritize being "cute" without striving for excellence.

In her adult incarnation in *Osomatsu-san*, Totoko retains her classic pigtails and headband but updates her wardrobe with a magenta turtleneck, plaid miniskirt, and high heels. Her selfishness and craving for admiration persist, driving her to become a "fish idol" to promote her family's fish shop purely for popularity and bragging rights. Her idol costume, incorporating real rotting fish, becomes a recurring burden due to its smell. Episodes like "Totoko's Big Panic" and "Totoko's Challenge" hint at fleeting self-awareness about her shallow behavior, though she consistently reverts to seeking validation for her cuteness. She remains acutely jealous of rivals and intolerant of criticism, insulting perceived "ugly" adversaries.

Her surname "Yowai," rarely referenced in early adaptations, was formally reintroduced in *Osomatsu-san*, notably appearing on an idol billboard.
