Grelle Sutcliff operates as a Grim Reaper in the Dispatch's Retrieval Division. Originally human, she died by suicide due to profound unhappiness from gender dysphoria and lack of acceptance, subsequently becoming a Reaper tasked with collecting souls. Her striking appearance features long red hair, chartreuse phosphorescent eyes, shark-like teeth, and red-framed glasses connected by a skull-adorned chain. She typically wears high-heeled ankle boots, a vest, gloves, and a distinctive red coat taken from Angelina Dalles after her death, frequently worn off the shoulders.

Her personality blends flamboyant theatricality with violence. She expresses herself unreservedly, often employing double entendres and referencing plays—likening relationships to "Romeo and Juliet" and deaths to "falling curtains." She exhibits a strong obsession with the color red, associating it with beauty and passion, and believes blood enhances beauty. Initially dismissive of Grim Reaper neutrality, her suspension led to increased protocol adherence, though impulsiveness still causes rule violations. She displays impatience with emotional hesitation, as seen when killing Angelina Dalles for refusing to murder Ciel Phantomhive. Sadomasochistic tendencies manifest in her attraction to violent individuals and her own use of playful violence.

Her weapon is a modified Death Scythe resembling a chainsaw, deemed unauthorized by Reaper authorities. This modification reflects her preference for brutality over efficiency, resulting in excessively gory killings. Her history includes posing as Angelina Dalles's butler, appearing timid and inept with brown hair and plain attire. She later conspired with Angelina as Jack the Ripper, targeting pregnant sex workers. She rationalized this partnership through shared trauma—Angelina's infertility and her own gender-related suffering. Their methods diverged: Angelina performed precise surgical incisions, while Grelle's attacks were brutally theatrical, using blood like makeup. When Angelina hesitated to kill Ciel, Grelle murdered her for perceived weakness, stealing her coat and declaring her "unfit for red." This violation of Reaper neutrality led to suspension after a confrontation with Sebastian Michaelis and intervention by William T. Spears.

Post-suspension, she returned during the Noah's Ark Circus case, eagerly collecting souls at Phantomhive Manor. Later, aboard the maiden voyage of the Campania, she partnered with Ronald Knox to investigate reanimated corpses ("Bizarre Dolls") and collect souls during the sinking. She prioritized soul reaping over investigation per Reaper principles, urging efficiency to avoid another suspension. During duties, she destroyed multiple Bizarre Dolls, accidentally flooded a hallway by slashing a window with her scythe, and battled Sebastian before redirecting focus to locating the dolls' creator, Rian Stoker.
