Tsukuyomi manifests as a young girl with long white hair and unsettling brown eyes that hold a wide, disturbing stare. She wears a black and white school uniform, contributing to an aura described as a "twisted adult trapped in a child's body." Her origins trace back to a crow rescued by Sarina Tendouji and Gorou Amamiya at their Miyazaki hospital. This crow watched over Sarina until her death before inhabiting its current human form—a "vessel" born to an unspecified mother Tsukuyomi deems abnormal. She hints her situation parallels the reincarnation of Aqua and Ruby, though her exact origins remain obscured.

She exhibits omniscient-like knowledge, including awareness of Aqua and Ruby's past lives, and claims responsibility for facilitating their reincarnation. Tsukuyomi can transfer souls of the deceased into newborns and commands crows, which frequently accompany her.
