Backward Cap Platelet, also called Ushiromae-chan or Backwards-Chan, serves as a platelet cell in the body's Platelet Division, performing injury response and blood clot formation. Her small, child-like appearance reflects the actual size of platelets; she possesses brown hair and brown eyes. A light beige cap worn backward marks her distinct look within the standard platelet uniform, which consists of an oversized blue T-shirt, light brown shorts, and black boots with white fur trim.

Formed by megakaryocyte fragmentation like all platelets, she works alongside fellow platelets under Megakaryocyte's guidance and frequently partners with Platelet Leader (Leader-Chan). Shy and clumsy, she often cries over minor setbacks like forgetting a sign or making small task errors. Despite this, she remains earnest and hardworking, frequently training alone to boost her strength and agility. She deeply admires Leader-Chan, seeing her as a role model and relying on her help during difficult situations.

Her journey focuses on overcoming insecurity. Introduced training solo in Season 2, Episode 1 ("Bump") after Megakaryocyte criticized her perceived weakness, she encounters Neutrophil (White Blood Cell U-1146) who encourages her to use her positive attitude. When a head injury ruptures capillaries causing zero-gravity chaos, she initially struggles to aid clot formation. Motivated by Megakaryocyte and Neutrophil's support, she risks her safety to retrieve a vital stray coagulation factor needed for the fibrin mesh. This critical act helps seal the injury, marking her progression from hesitancy to a determined team member embracing collaboration.

Her capabilities match standard platelet functions: achieving hemostasis through aggregation using Von Willebrand Factor, forming clots via fibrin nets, and creating thrombi by binding blood cells. Lacking combat skills, platelets depend on white blood cells for emergency protection. She strictly follows Platelet Division rules: staying with her group, avoiding internal conflicts, and using adhesion proteins like GPIb to withstand blood flow during wound responses.

Across her appearances, relationships emphasize her role within the platelet community. She shares close bonds with Leader-Chan and other platelets, operating in coordinated groups for tasks such as cargo transport or stair navigation.
