He gained esper abilities three years before transferring into North High School, leading to his recruitment by the Agency—which some sources suggest he may have founded. Haruhi recruited him into the SOS Brigade as her ideal "mysterious transfer student" shortly after expressing a desire for such a figure, implying her powers influenced his arrival. Within the Brigade, Haruhi promoted him to Deputy Chief after he orchestrated a staged mystery during the Remote Island Syndrome incident.

In the altered reality of *The Disappearance of Haruhi Suzumiya*, created by Yuki Nagato, Koizumi exists as a normal human without esper abilities. He attends Kouyouen Academy, a co-ed school, and openly confesses to harboring romantic feelings for Haruhi Suzumiya.
