Description
Ryuji Hirasaka, a middle-aged Japanese gynecologist, combines a depraved intellect with manipulative cunning. Initially unemployed and self-reliant, his career is marred by unethical acts, including the brutal rape of Narumi Jinguji—a colleague who later ascends to lead St. Juliana Hospital. Despite this history, Narumi enlists him to establish a hospital division addressing patients’ sexual desires, capitalizing on his skill in psychological exploitation.

Specializing in cold reading, Ryuji identifies vulnerabilities in nurses and staff, coercing them through blackmail and psychological torment. He weaponizes personal secrets—malpractice scandals, illicit affairs—to enforce obedience, pushing subjects into participating in his unethical experiments. Nurse Ren Nanase, initially cordial, becomes a prime target: after uncovering her match-fixing past, he subjects her to methodical abuse, culminating in her Stockholm syndrome-driven attachment.

His narrative diverges across media. The primary storyline concludes with Ren reclaiming autonomy, stabbing him to death in episode 5. Yet spin-offs like *Night Shift Nurses: Kranke* depict an alternate fate: surviving, marrying Ren, and relocating to a rural clinic where he perpetuates experiments under medical pretense. Here, he manipulates others, including Hikaru Kodama and her adoptive sister Ai, disguising self-interest as benevolence.

The prequel *Yakin Byōto Zero* traces his origins to a medical school rivalry with Narumi, who challenges him to seduce specific women before graduation. This rivalry ignites his obsession with psychological control, cementing manipulation as his primary tool.
