Misa Nakamura serves as a servant on the island. Her most significant relationship is with Kasumi Ashihara, her lover. Their intimacy includes a scene where Kasumi forces Misa against a wall and performs sexual acts on her in an open household area.

Her primary role involves the island's cult rituals. Selected early as a sacrificial victim, during the ceremony Kasumi continues to pleasure Misa before surrendering her to the island priest. The priest rapes Misa ritualistically while Kasumi commands her to reach climax as an offering to Master Mizuno. Kasumi then stabs Misa with a sword, killing her. Misa's blood flows across the chamber, nourishing the island's red flowers that bloom from such sacrifices.

Following her death, the cult harvests Misa's muscle tendons for constructing mannequins as part of the island's dark practices. Her beating heart is presented during a purification ceremony the next day.
