Hisako Arato is a student of the 92nd Generation at Totsuki Culinary Academy, primarily known for serving as the personal secretary and close aide to Erina Nakiri. She is a young woman with short pink hair, typically styled in a bob with bangs that reach her chin, and reddish-pink eyes; she sometimes wears glasses when performing administrative tasks. Born into the Arato family, a household renowned for its mastery of traditional Chinese medicine, Hisako is a specialist in medicinal cooking, or药膳 (yàoshàn). She skillfully integrates medicinal herbs, spices, and ingredients like ginseng into her dishes, aiming to create food that is not only delicious but also nutritionally balanced and beneficial for health.

Her personality is defined by an unwavering loyalty and devotion to Erina Nakiri, whom she has known since childhood and views as her Empress. Hisako's primary motivation is to use her expertise in medicinal cuisine to serve as Erina's personal food doctor, managing her health and allowing her to focus on her culinary ambitions. She is highly efficient and organized, demonstrating impressive secretarial skills by managing Erina's schedule, tasks, and correspondence. Initially, she shares Erina's prideful and dismissive attitude towards others, particularly those who pose a perceived threat to Erina's status, such as the protagonist Soma Yukihira. She often belittles others in Erina's stead and is fiercely protective of her position.

In the story, Hisako's role is initially that of an extension of Erina, monitoring Soma's progress and attempting to undermine his place at the academy. However, her character undergoes significant development during the 43rd Annual Totsuki Autumn Election. After winning her preliminary round against Nao Sadatsuka, a rival for the secretary position, she is defeated in the quarterfinals by Akira Hayama, a specialist in spices. Hayama criticizes her for lacking her own ambition and for being content as Erina's number two. This loss deeply shakes Hisako's confidence, leading her to believe she is no longer worthy to stand by Erina's side, prompting her to temporarily distance herself.

Her path to recovery begins during a stagiaire, or external internship, where she is paired with Soma Yukihira at the Western-style restaurant Mitamura. Initially hostile towards him, she witnesses his relentless work ethic and adaptability. More importantly, Soma offers her crucial advice, encouraging her to stop following behind Erina and instead find a way to stand beside her as an equal. This conversation helps Hisako regain her self-worth and resolve, leading her to return to Erina with a renewed purpose. She transitions from a self-defined servant to a true friend and partner, declaring later that she is Erina's friend rather than just a lowly aide. This marks a turning point, and she becomes much more open and friendly, even developing a strong friendship with Soma, much to Erina's surprise.

Hisako's key relationships are central to her narrative. Her bond with Erina Nakiri is the most important, evolving from one of pure subservience to a mutual friendship where she walks alongside her. Her relationship with Soma Yukihira transforms from antagonistic to supportive, as he plays a pivotal role in her personal growth. Her interactions with Nao Sadatsuka are also notable; after defeating Nao in a Shokugeki for the secretary role, Nao develops an obsessive admiration for Hisako, adding a layer of comedic unease to their rivalry. Her notable abilities include her profound knowledge of Chinese medicine, which she applies to her culinary style, and her exceptional organizational and managerial skills, which are as sharp as her cooking talent.
