At the beginning of the episode, Yuji Itadori finds himself in a dilemma: torn between saving the people at the train station and searching for Gojo. He asks Inumaki for help, using their unique coded language. The setting is Shibuya Station, and the atmosphere is tense.

Choso, who has identified Yuji as the killer of his brothers, confronts him. Choso uses a technique called "Blood Manipulation," specifically an ability named "Convergence," to compress and release blood at high speed. Yuji finds it difficult to dodge these high-speed attacks and decides to control the timing himself. He discovers that the attack loses speed after the initial release, allowing him to counterattack.

Mechamaru intervenes in the conversation and provides crucial information about Choso's technique. He compares it to Kamo's technique and explains that it is an inherited ability of the Kamo clan. Mechamaru proposes a daring plan: Yuji should lure Choso into the restroom, where the water could disrupt his Blood Manipulation.

Choso follows Yuji but remains cautious. He senses that Yuji is up to something but can't figure out what. Once in the restroom, Yuji's plan succeeds. The water causes Choso's blood cells to expand and burst, interrupting his "Convergence" technique. Mechamaru's advice proves correct, and Choso is temporarily unable to use his technique outside his body.

However, Choso quickly adapts. He uses another form of Blood Manipulation, "Blood Meteorite," to severely injure Yuji. Yuji realizes the gravity of the situation but remains determined to defeat Choso, even if it costs him his life. Choso also recognizes Yuji's strength and decides to take a risk by clotting his blood, despite the potential danger of thrombosis.

In a final clash, both fighters give their all. Choso falls for a feint by Yuji, who holds back his left fist. As Choso prepares for the final strike, he is overwhelmed by a flood of nonexistent memories and calls Yuji "brother."

The episode ends with two mysterious figures confirming Yuji's survival and speaking about the beginning of something new. The next episode is titled "Fluctuations."
