A quiet day at Ouran Academy’s Host Club is disrupted when the eccentric members discover it is Haruhi Fujioka’s birthday. The club, composed of the princely Tamaki Suoh, the calculating Kyoya Ootori, the mischievous Hitachiin twins Hikaru and Kaoru, the sweet-toothed Honey-senpai, and his stoic companion Mori-senpai, immediately decides to celebrate. However, their elaborate and over-the-top plans, fueled by a fundamental misunderstanding of what Haruhi would actually enjoy, quickly spiral into chaos.

The hosts, accustomed to a world of grand gestures and extravagant wealth, propose a series of increasingly absurd surprises. They fail to realize that Haruhi, a scholarship student focused on her studies and practical matters, values simplicity and thoughtfulness over spectacle. The well-intentioned but misguided efforts lead to a series of comedic mishaps and hurt feelings. Ultimately, the collective failure to listen to Haruhi results in the club members accidentally making her very angry, causing her to storm out and leave the Host Club in a rare moment of open frustration.

Left behind in the music room, the hosts are forced to confront their own assumptions about Haruhi and what it truly means to be a thoughtful friend. They must devise a genuine plan, free from their usual flair for the dramatic, to find her and properly make amends. The special focuses on the resulting antics as the hosts scramble to locate Haruhi and prove they can celebrate her in a way that honors who she actually is, not the fantasy version they have in their heads.
