Her sentient existence emerged approximately ten years before the main events, coinciding with Kamijou Touma's arrival in Academy City. Aleister Crowley deliberately introduced the Imagine Breaker to stabilize and control the volatile Imaginary Number District, granting it conscious form through her. She manifests as a teenage girl possessing long dark brown hair, frequently styled in a side ponytail secured by a hair bobble, and distinctive golden eyes. Her attire consists of the standard Kirigaoka Girls' Academy uniform, featuring a blazer, skirt, and red necktie, later supplemented by a blue jacket for winter. Large glasses often slide down her nose, and her physical form includes a notably large bust. Gentleness, shyness, and a propensity to tear up easily define her personality. Significant timidity marks her initial demeanor, especially around Touma due to a subconscious fear that his Imagine Breaker could negate her existence—an experience she likens to static electricity on wool. During a subsequent trip to Academy City's Underground Street with Touma and Index, the magician Sherry Cromwell attacked, targeting Hyouka as the "Key" to incite war between science and magic. When Sherry's golem, Ellis, injured Hyouka, it revealed her non-human physiology—her head proved hollow, containing a triangular prism core. This discovery plunged her into despair, intensified when Sherry branded her a monster. Touma defended her in the ensuing battle, declaring her his friend regardless of her nature. Anti-Skill provided suppressive fire against Ellis, enabling Touma to confront Sherry. Resolving to protect Index alone, Hyouka leaped into a subterranean tunnel, using herself as bait to divert Ellis. Though injured, her regenerative abilities healed her. She reached Index just as Ellis attacked again, blocking the assault but beginning to destabilize physically. After Touma intervened once more, Hyouka retreated, convinced her "illusion" of humanity had shattered. Touma later found her and reassured her that her capacity for pain and sadness proved her humanity. Index's arrival further solidified their bond.
