Motoharu Tsuchimikado is Kamijou Touma's next-door neighbor and classmate at Academy City's high school, part of a comedic trio dubbed the "Three Idiots" or "Delta Force" with Aogami Pierce. Initially presented as an eccentric side character with dyed blonde hair, permanent sunglasses, a gold chain, and Hawaiian-style shirts, his role deepens significantly as a multi-faceted operative.

His background reveals he was originally a high-ranking magician for the Anglican Church's Necessarius, specializing in Onmyoudou and Feng Shui, skills he taught to others like Kanzaki Kaori in London. To infiltrate Academy City as a Necessarius spy, he underwent the Power Curriculum Program, becoming a Level 0 esper with the ability "Auto-Rebirth," which seals torn blood vessels and repairs bodily damage. This transformation makes using magic life-threatening due to esper-magic incompatibility; casting spells causes severe internal injuries, bleeding from bodily orifices, and carries a finite risk of death, with Tsuchimikado estimating only 5–6 "lifelines" remaining. His infiltration was discovered within 36 hours, forcing him to become a double, then triple agent for Academy City's board, Necessarius, and the underground group "GROUP" to survive.

His key motivator is devotion to his stepsister, Maika Tsuchimikado, adopted from an orphanage outside Academy City to create a fake family for his cover. Initially viewing her as part of his mission, he grew genuinely protective after recognizing her kindness, vowing to shield her at all costs. This fierce loyalty defines his actions, earning him the nickname "Siscon Sergeant" for his obsessive doting and jealousy when others earn her affection. His personality balances a cheerful, manipulative trickster persona—self-described as "a big fat liar" who betrays freely—with cold pragmatism during missions. He maintains friendships with Touma and classmates while undertaking ruthless operations, including assassination attempts, to preserve the balance between science and magic factions. He blames himself for failures like the Roman Catholic Church–Academy City conflict, viewing his role as a behind-the-scenes stabilizer.

In combat, he relies on hand-to-hand techniques due to magic's physical toll, employing "Shitotsu Satsudan" (Deadly Thrust Killing Slash), a martial arts style incorporating dirty tactics that enabled him to defeat Touma effortlessly. As an Onmyoudou master, he uses origami charms for spells like long-range energy blasts ("Beam of Light"), barriers, speed enhancements, and planetary-scale defensive shields, though each use risks catastrophic self-harm. His strategic intellect and espionage experience make him a formidable planner, often manipulating allies and enemies alike under the magical name "Fallere825" (The Backstabbing Blade).

His development shows escalating sacrifices. During the "Angel Fall" arc, he assists in dismantling the ritual despite severe injuries. Later, as a core member of GROUP, he undertakes black-ops missions for Academy City while juggling loyalties. In *New Testament Volume 7*, he suffers a psychological collapse after a false report of Maika's death, leading to a violent rampage and subsequent defeat by Touma. Presumed dead afterward, he is secretly smuggled out of Academy City by Seria Kumokawa, faking his demise to protect Maika. Outside the main series, he appears in spin-offs like *A Certain Scientific Railgun*, often in lighthearted contexts such as recommending absurd costumes to Maika.
