Kaoru Akashi wields Level 7 psychokinetic abilities, placing her among the most powerful documented espers. Her telekinesis manipulates objects, controls directional force with pinpoint accuracy to ignite fires, negates gravity vectors for flight, and halts massive objects like boulders. A limiter bracelet typically restricts her to Level 4, but deactivation unleashes her full power. She creatively names techniques such as "Psycho Drill Crusher" and "Psychic Megaton Punch."

Neglected in childhood by her mother Akie Akashi and sister Yoshimi Akashi following an incident where her uncontrolled powers hospitalized her mother, Kaoru developed self-loathing towards her abilities and existential questioning. Early isolation ended when B.A.B.E.L. chief Taizō Kiritsubo introduced her to fellow Level 7 espers Aoi Nogami and Shiho Sannomiya. A prophecy foretold by psychic dolphins I-008 and I-009 predicts Kaoru becoming the "Queen of Catastrophe," leading the esper faction P.A.N.D.R.A. in a devastating war against normal humans. This future culminates in her death by Minamoto's hand after she manipulates his weapon via psychokinesis, confessing her love with her final breath. Kyousuke Hyoubu, witnessing this prophecy during World War II, becomes obsessed with ensuring this outcome, dubbing her "Queen" and positioning her as P.A.N.D.R.A.'s future leader.

Her development shows progression: Elementary school saw her resist missions until provoked, exhibit poor emotional control, and destroy robots spewing anti-esper rhetoric. By high school, she temporarily assumed P.A.N.D.R.A. leadership under Hyoubu's directive during his incapacitation. She publicly denounced the organization's methods in a viral video, declaring war on the entity "Gilliam" while upholding Minamoto's ideals of human-esper harmony. This strategic acceptance of her "Queen of Catastrophe" title aimed to prevent the prophesied future.

Key relationships define her actions: Her primary affection since age ten is Kōichi Minamoto, whom she views as a guardian, protecting him fiercely; future iterations affirm intensified love. Minamoto reciprocates deep concern, carrying the weapon fated to kill her while striving to avert the prophecy. Hyoubu cultivates her as P.A.N.D.R.A.'s successor, addressing her as "Queen"; Kaoru generally disregards his provocations but later collaborates tactically. Despite early conflicts with Aoi and Shiho, they form an unbreakable team; future predictions show initial opposition to Kaoru's rebellion, though she eventually sways them by citing B.A.B.E.L.'s esper mistreatment.

Efforts by "Feather" (a future Kaoru) to alter the catastrophic future successfully erased the original timeline. However, threats like Gilliam resurfaced, compromising both B.A.B.E.L. and P.A.N.D.R.A. and forcing Kaoru's renewed leadership role. She maintained Minamoto's teachings throughout, leveraging her position to pursue coexistence.
