Description
Shinra Kishitani operates as an underground doctor in Ikebukuro, invariably clad in his signature white coat regardless of context. His unconventional medical education began early under his father, Shingen Kishitani, who involved him in surgeries from childhood. This culminated at age four with Shinra participating in the vivisection of Celty Sturluson, discovered on a ship. The procedure forged a lasting arrangement: Celty became his roommate in exchange for housing.

During primary school alongside Shizuo Heiwajima, Shinra developed a fascination with Shizuo's supernatural strength, theorizing his bones fractured under stress only to heal stronger. He later befriended Izaya Orihara in middle school, co-founding a biology club that Izaya transformed into a gambling ring. An incident saw Shinra intercept a knife meant for Izaya, then falsely claiming Izaya stabbed him as requested. In high school, Shinra introduced Izaya to Shizuo, igniting their enduring feud. He retrospectively blames both for disrupting his high school years, though their conflicts honed his orthopedic skills.

Shinra exhibits an eccentric personality detached from humanity, contrasting Izaya's professed love for humans. He expresses no romantic or sexual interest in humans, stating he would be asexual without Celty. His obsessions fixate on supernatural phenomena, particularly Celty's physiology and Shizuo's abilities. He possesses an exceptional aptitude for interpreting Celty's emotions despite her lack of facial expressions. Their relationship forms his core, defined by mutual devotion and insecurity. Fearing Celty's departure should she recover her missing head, Shinra performed plastic surgery on Mika Harima to resemble it, deliberately obstructing Celty's search. Discovered, Celty assaulted him but they reconciled after he affirmed her intrinsic value without the head.

Operating without a license, Shinra frequently treats injuries sustained by acquaintances. Notable patients include Shizuo after gunshot wounds, Ruri Hijiribe due to her rapid healing, and Egor at Russia Sushi. He imposes exorbitant fees when inconvenienced, demanding 200,000 yen from Mairu and Kururi Orihara for emergency care.

In expanded narratives, Shinra investigates supernatural occurrences like the demon blade Saika, debunked as a modern replica. He becomes entangled in Project Tadpole through his father's research into creating "ultimate witnesses" for apocalyptic events, accessing confidential data on subjects like Mikado Ryuugamine while concealing this from Celty. He later survived a fatal shooting by Goyoku after temporary death experiences in purgatory, where entities restored him to his body. Post-resurrection, persistent reapers target his soul. Additional events include assisting Celty's shadows to exorcise Seiji Yagiri and studying the dangerous Book of Enma under duress, which inflicted traumatic visions.

Shinra's childhood trauma includes maternal abuse documented in Shingen's journals, explaining his emotional detachment from humans and preference for Celty's headless presence. A formative incident occurred when Celty attended his elementary school observation day disguised with a mannequin head, cementing her role as his primary familial figure.
