Description
Miyoko Sahara first appears as Shoko Nishimiya and Shoya Ishida's elementary school classmate. She becomes the only student willing to learn Japanese Sign Language to communicate with Shoko, forging a genuine friendship. This association makes Miyoko a target of bullying, particularly from Naoka Ueno, leading her to withdraw from regular classes to the nurse's office and eventually transfer schools before Shoko's own transfer due to escalated bullying. Miyoko remains unaware of these subsequent events for years.

Physically, Miyoko is notably tall at 5'11" (180 cm), with a slender build, freckles, gentle eyes, and short black hair. By high school, her height and model-like physique attract admiration. She typically wears her school's sailor uniform and favors dark or black clothing with high heels casually.

Personality-wise, Miyoko is fundamentally kind-hearted, empathetic, and loyal, especially to Shoko. She maintains their friendship through activities like karaoke despite Shoko's tone-deaf singing. However, she exhibits a passive, non-confrontational nature and a tendency to avoid conflict—a flaw Shoya Ishida criticizes during an argument, accusing her of being "quick to run." Miyoko acknowledges this and actively works to overcome it, showing gradual progress in confronting difficult situations.

In high school at Taiyō Girls Academy, Miyoko studies fashion design. Her height and physique lead to runway modeling becoming a hobby. She unexpectedly reunites with Naoka Ueno, now a classmate. Despite initial tension from their past, they form a complex friendship. Miyoko defends Naoka's design work from criticism, leading to collaboration. Miyoko also reconnects with Shoko through Shoya's efforts, resuming their friendship and joining group activities like trips to a waterpark and working on Tomohiro Nagatsuka's film project. She continues practicing sign language and supports Shoko emotionally, often facilitating challenging conversations.

Her relationship with Naoka deepens despite conflicts, such as Miyoko intervening to stop Naoka from attacking Shoko. Their professional collaboration peaks when they win a fashion design contest with a costume for Nagatsuka's film, resulting in acceptance into a Tokyo college. After graduation, Miyoko and Naoka move to Tokyo together, establishing a fashion brand. The narrative hints at a deepening bond, including Miyoko giving Naoka a ring and them living together, suggesting a potentially romantic relationship.

Miyoko reconciles with Shoya after his critical outburst. She later thanks him for helping her reconnect with Shoko and becomes part of his friend group, participating in regular meetups. When Shoya is hospitalized following a suicide intervention involving Shoko, Miyoko reflects on his earlier criticism, recognizing his growth while feeling inadequate about her own efforts to change. They eventually apologize to each other, with Shoya affirming that her attempts to improve matter more than immediate success.

Two years after high school, Miyoko returns to Suimon for the coming-of-age ceremony. She maintains connections with her friends and attends an elementary school reunion with the group, continuing her journey of personal growth.
