Sara, a female doctor linked to the Hokuto Shinken dojo, encounters Toki during his travels as a healer in the nuclear war's aftermath. Drawn by his reputation for compassion and using martial arts for healing, she formally diagnoses his failing health as terminal radiation sickness—a consequence of his sacrifice protecting others during the catastrophe.

Recognizing his condition, Sara chooses to accompany Toki on his wasteland pilgrimage. She aids him in treating the sick and injured, her medical expertise complementing his therapeutic Hokuto Ujō Ken techniques. She remains a constant companion throughout his journeys, witnessing his unwavering commitment to ease suffering despite his own decline.

Sara is present as Toki confronts his brother Raoh, fulfilling a childhood promise to stop Raoh's straying from the righteous path. After Toki survives the battle, he returns to her side. Sara continues her supportive role through Toki's final days, providing dedicated care and companionship until his death.
