Lilith operates as a demonic entity aligned with Atun, collaborating with antagonist Dr. Adams. She emerged in astral form near a convenience store, identifying and possessing a human woman. This possession reshaped the host's body to match Lilith's demonic physiology while transforming nearby humans into subservient slaves under her command.
Her form blends botanical and humanoid traits. Her legs fuse into a single vine-like tail, and dark leaf-like structures cover her shoulders and upper body. Arms split into two segments below the elbows. Her breasts feature faces, each possessing a single red-sclera eye and a mouth filled with sharp teeth.
