The sequel manga Busa-men Gachi-Fighter SSS follows the continuing adventures of Shigeru Yoshioka, a man who was reborn into a fantasy world after his life as a salesman in Japan was ruined by a false accusation. To gain immense power in this new world, he made a drastic sacrifice, setting his appearance stat to an abysmal negative 255. In exchange for becoming an unsightly, middle-aged shut-in, he was granted cheat-level abilities that make him an Absolute God. However, this power comes with debilitating curses: any physical contact with a woman drastically reduces his hit points, and any sexual act would result in instant death.

The story takes place after the conclusion of the original manga. Shigeru and his companions—the handsome but chuunibyou-infected hero Seiji, the kind and naive mage Seika, and the mysterious, young-looking hunter Leeds—have founded a company called Visbridge. While operating under the guise of a temporary staffing agency, the group secretly works to defeat evil organizations and support other troubled individuals who have been transported to their world. Their peaceful mission is disrupted by the emergence of a new threat: another Absolute God whose power rivals Shigeru’s own. As they investigate this powerful enemy, the narrative also delves deeper into the hidden past of their quiet and perceptive ally Leeds, whose secrets become central to the unfolding conflict.

Several key characters drive the story. Shigeru Yoshioka, the protagonist, is an ugly man with a heart of gold and a strategic mind, who prefers to be called a Gachi-Fighter despite his godly power. Seiji is the passionate and justice-seeking leader of the group who can transform into the invincible Aldeen. Seika is the party's innocent and caring healer, who transforms into Elcalone the Sun Saint. Leeds is a highly capable fighter who can read other people’s stat panels; her true identity as Seiji’s younger sister and her past are major plot points. The sequel arc notably introduces the concept of an SSS Rank, which represents the lowest possible appearance, and therefore, the highest possible combat power, creating a society where the ugliest individuals are the most physically formidable.
