Description
Kuroo Hazama, known as Black Jack, endured childhood trauma when a landmine explosion on a beach killed his mother and left him severely injured. Dr. Jotaro Honma saved him through extensive surgery, utilizing skin grafts from a biracial classmate named Takashi. This resulted in his distinctive patchwork appearance with contrasting skin tones and black-and-white hair. The event inspired his pursuit of medicine, though he harbored lasting resentment toward his father, Kagemitsu Hazama, who abandoned the family after the incident.

As a student, he became Dr. Honma's top pupil but developed a rebellious streak, skipping classes and isolating himself due to his unsettling appearance. A friendship with a schoolmate helped him regain his ability to smile. He later performed an unauthorized surgery on his romantic interest, Megumi Kisaragi, to remove cervical cancer, saving her life but costing him his medical license for using unapproved methods. This led him to establish an independent practice in a cliffside home.

He lives with Pinoko, a sentient teratogenous cystoma he extracted from a patient and housed in a synthetic body. He views her as an adoptive daughter, though she insists they are married. His pet dog, Largo, accompanies them in some adaptations. Operating as an unlicensed surgeon, he charges exorbitant fees—often millions—from wealthy or corrupt clients but treats the impoverished or morally deserving for free. His fees fund environmental conservation efforts, including purchasing islands to prevent development.

His personality blends apparent callousness with deep compassion. He cultivates a Dr. Jerk persona to deter social connections but demonstrates unwavering dedication to patients, accepting barter or waiving fees based on moral assessments. He employs Batman Gambits to teach humility to arrogant patients or corrupt officials, such as inflating fees for the unethical or deceiving patients into compliance for their recovery. He is fiercely protective of Pinoko and child patients, exhibiting Papa Wolf tendencies. His surgical precision extends to combat, where he throws scalpels with dart-like accuracy, a skill honed in his delinquent youth.

Ethically complex, he operates without a license and occasionally uses experimental methods, drawing criticism from the medical establishment. Yet, he refuses to join corrupt systems and pursues personal justice, hunting down those responsible for his mother's death. He rejects the Hippocratic Oath due to this vendetta but adheres to a personal code: honoring agreements, repaying debts, and ensuring patient survival at all costs, including self-surgery in life-threatening scenarios. His scarred face, black cape, and red or blue bowtie intentionally evoke intimidation, though many find him magnetically attractive despite it.

Key relationships include his unresolved bond with Megumi (later Kei Kisaragi), who transitioned to a male identity post-surgery and expressed enduring love for him. He harbors a rivalry with the euthanasia-focused Dr. Kiriko. In spin-offs like Black Jack 21, he confronts his father, whose Phoenix Project microchip was hidden in a necklace from Black Jack's mother, culminating in a skin graft from his father's corpse during a leg surgery—symbolizing fractured reconciliation.

Medically, he is termed "a surgeon with the hands of God" for impossible feats: transplanting limbs without rejection, operating in darkness using touch alone, and treating non-human entities like aliens or animals. His diagnostic skills allow him to assess conditions at a glance, and his resilience ensures he never abandons patients, even amid danger.
