Description
"Chaos;Head" is a psychological thriller anime based on the visual novel by 5pb. and Nitroplus. The story centers on Takumi Nishijou, a socially isolated high school student who spends most of his time immersed in delusions and playing online games. His life takes a dark turn when he becomes entangled in a series of gruesome murders known as the "New Generation" incidents. These killings are marked by bizarre and inexplicable phenomena, drawing the attention of both the public and law enforcement.

Takumi's involvement begins when he encounters a mysterious girl named Rimi Sakihata, who warns him about impending danger. Shortly after, he receives disturbing images on his phone that seem to predict future events, including the murders. As the story progresses, Takumi discovers that these images are linked to a phenomenon called "Di-Sword," a manifestation of one's delusions into a physical weapon. The Di-Swords are tied to a secretive organization known as NOZOMI, which conducts experiments on individuals with heightened delusional abilities, referred to as "Gigalomaniacs."

The narrative explores the blurred lines between reality and delusion, as Takumi struggles to discern what is real and what is a product of his unstable mind. His interactions with other characters, such as the enigmatic Yua Kusunoki, the stoic Kozue Orihara, and the cheerful but secretive Sena Aoi, further complicate his understanding of the events unfolding around him. Each character has their own connection to the New Generation incidents and the experiments conducted by NOZOMI.

As Takumi delves deeper into the mystery, he uncovers the truth about his own past and the origins of his delusional abilities. The story builds toward a climactic confrontation with the forces behind the New Generation murders, revealing the extent of NOZOMI's manipulation and the true nature of the Di-Swords. Yua Kusunoki enters Takumi Nishijō's classroom. Takumi screams and runs away. Rimi Sakihata finds him near the school courtyard. Takumi agrees to let Rimi support him,and the two begin walking to and from school together. After classes end, Takumi and Rimi go to a café. Takumi tells Rimi about his encounter with the person called Shogun. A protest march against the New Generation events passes outside the café window. Back at his container home, Takumi holds the counterfeit Di-Sword and attempts to summon a real one. Rimi stops him before he can complete the attempt. Rimi leaves to get more soda. While she is gone, Takumi learns that his former psychiatrist, Fumio Takashi, has died as the victim of the fifth New Generation incident. Elsewhere, Sena Aoi destroys a device manufactured by the Nozomi Technology Group. The device was hidden inside someone's backpack. The next day, Ayase Kishimoto instructs Takumi on how to find his Di-Sword. After school, Rimi walks Takumi home. Takumi tries to tell Rimi that he may possess the ability to turn delusions into reality. Rimi does not believe him. Detective Ban visits Katsuko Momose to gather information about the New Generation events. Takumi receives a phone call from Shogun. When he answers, he hears only a strange electronic tune, the same sound emitted by Shibuya's pedestrian crossings. A powerful earthquake suddenly strikes Shibuya. A loud alarm fills the air, and the sky turns white.
7Realization
Following the earthquake,Takumi arrives at school searching for Rimi. Ayase jumps from the school roof, but Takumi unconsciously real-boots a flowerbed that breaks her fall. Rumors spread through the school that Takumi's relationship with Ayase drove her to attempt suicide, and his classmates begin teasing him. A feminine voice starts speaking inside Takumi's head. After school, hired thugs beat Takumi in an alley. Kozue, a classmate who communicates telepathically, rescues him and defeats the attackers. Kozue reveals she possesses her own Di-Sword. Sena appears and explains that individuals who can see and wield Di-Swords are called Gigalomaniacs. Gigalomaniacs possess the power to project their delusions into reality through a process called real-booting. The Nozomi Technology Group operates a machine named Noah II that synthetically amplifies Gigalomaniac powers for its own purposes. The backpack devices Sena destroyed in the previous episode serve as mobile transmitters that broadcast Noah II's signals. Takumi learns he can real-boot delusions without consciously summoning a Di-Sword, making him unique. Noah II generates a delusion of a runaway digging machine that charges through the streets, causing mass panic. Sena and Kozue draw their Di-Swords and destroy the mobile transmitters carried by nearby Porters, dispersing the illusion. The Nozomi Group vows to eliminate all obstacles to its plan. Sena, Kozue, and Takumi agree to form a group to stop Nozomi, and Kozue insists they take a commemorative photo together in a photo booth. Sena warns Takumi that his uncontrolled real-booting ability makes him a target for Nozomi's forces.
8Escape
Takumi Nishijou wakes up strapped to a hospital bed in a research facility. Scientists inject him with a drug that forces his right arm to materialize a Di-sword. Rimi Sakihata appears in his room and cuts his restraints with her own Di-sword. She explains that the facility belongs to the Noah II project,which studies Gigalomaniacs. Takumi and Rimi flee through the corridors, pursued by armed guards. Takumi's delusions manifest as barriers and false walls, allowing them to evade capture. They reach a large chamber containing a massive spherical object, the Noah II. The lead researcher, Norose, activates the Noah II, which begins emitting waves that disrupt Takumi's delusions. Rimi confronts Norose, but she hesitates to strike him. Takumi's sister Nanami arrives with a group of escapees, creating a diversion. In the chaos, Takumi grabs Rimi and runs toward an emergency exit. Norose orders the guards to shoot, and a bullet grazes Rimi's shoulder. Takumi drags her through the exit door, and the building's alarm system seals the entrance behind them. Outside, they find themselves in a forest, but Rimi collapses from blood loss. Takumi looks back at the facility, now hearing a voice from the Noah II calling his name.
9Rejection
Yua Kusunoki approaches Detective Ban and confirms she is the twin sister of the first New Generation victim. Ban receives a report of a sixth New Generation incident where three bodies are found mutilated. Takumi learns online that the three victims are the same individuals who attacked him in episode seven. Takumi visits Ayase Kishimoto in the hospital,where she tells him he is one of the seven knights and must obtain a Di-Sword. Returning to his trailer with Rimi, Takumi finds Sena Aoi waiting for them on the roof. Sena attacks Takumi, but Rimi blocks the strike by summoning her own Di-Sword, revealing herself as a Gigalomaniac. Rimi forces a delusion onto Sena, making her see her father and freezing her attack. Sena explains that Takumi drew the formula "Ir2" as a child, which Nozomi used to develop the Noah II device that creates artificial Gigalomaniac powers. Sena also states that Noah II killed her mother and sister. Rimi declares her mission is to prevent Takumi from awakening. Takumi receives a call from his sister Nanami pleading for help, but he dismisses it as a prank and hangs up. A hospital nameplate reveals that Shogun's room bears Takumi Nishijo's name, raising the question of the true relationship between Takumi and Shogun.
10Blast Off
Takumi Nishijou pulls the Di-Sword from his chest in the hospital corridor. The blade solidifies from his delusions and cuts through a metal railing. Rimi Sakiharu confirms that all Gigalomaniacs can manifest their delusions as reality. Takumi still doubts his own power until he deflects a bullet from a Nozomi agent. The group escapes the hospital and heads to the school rooftop. Sena Aoi draws her own Di-Sword and blocks a missile from the Noah II weapon system. Kozue Orihara collapses from exhaustion after maintaining a defensive delusion field. Yua Kusunoki fires a handgun at the approaching mecha,but the rounds ricochet off its armor. Takumi steps forward and swings his sword at the Noah II's central core. The explosion destroys the mecha and sends everyone to the ground. Nanami Nishijou arrives and shouts that the real enemy remains hidden. Professor Norose emerges from the smoke behind the group, holding a remote detonator linked to the city-wide delusion amplification system.
11Independence
Takumi meets Shogun face to face in the ruined streets of Shibuya. Shogun explains that he created Takumi as a delusion to stop Nozomi from abusing the Ir2 equation with Noah II. Rimi allowed herself to be captured while trying to rescue Nanami. Takumi recalls how Shogun freed Rimi from her darkness in the past using a synchronized delusion. Recognizing his love for Rimi,Takumi summons his Di-Sword for the first time. Shogun directs Takumi to destroy the Porter devices causing chaos in the pedestrian scramble. Takumi destroys multiple Porters using his awakened powers. Hazuki Shino appears as the final Porter and challenges Takumi to read her mind. Takumi projects her memories onto the large building screens, exposing her as the true New Generation criminal. Shino stabs herself in the neck with a cross-shaped stake, triggering the Third Melt, a magnitude 7.8 earthquake that levels Shibuya. Takumi regains consciousness amid the wreckage and uses his Di-Sword to rescue Ayase, Yua, and Nanami from a collapsed pit. Takumi tells Nanami about his delusional existence and reunites her with Shogun, her real brother. Sena and Kozue encounter Sena's father Hatano in a subway tunnel. Suwa appears and shoots at Sena, but Hatano jumps in front of the bullet and dies protecting her. Suwa attacks Sena and Kozue with delusions, but Takumi arrives and reverses the attacks. Takumi kills Suwa by swapping their positions with a manipulative delusion, turning Suwa's own stakes against him. Sena tearfully asks Takumi to destroy Noah II, and Takumi agrees, continuing his advance to save Rimi and destroy the Noah II system.
12Mission
Takuya accepts his identity as a Gigalomaniac and manifests a real Di-Sword from his chest,cutting through Norose's illusionary world. Norose activates the Noah II terminal to project his own delusion, intending to overwrite Tokyo's reality with a utopia controlled by his will. Rimi resists Norose's mental binding and injects him with a sedative, temporarily breaking his concentration and freeing Takuya from paralysis. Takuya envisions Norose as an elderly man decaying rapidly, and the delusion physically ages Norose into crumbling dust within seconds. The Noah II machine overloads from the conflicting delusions and explodes, releasing all accumulated energy into the sky above Shibuya. The earthquake that had been tearing apart the city stops immediately, and the collapsing buildings freeze mid-destruction before restoring to their original states. Sena, Kozue, Yua, and Ayase wake up from their comatose states, their Di-Swords dissolving as the delusion field collapses. Takuya expends all his remaining delusion energy in the final attack and loses his Gigalomaniac powers permanently. The group parts ways, and Takuya returns to his solitary life inside the shipping container, accepting his ordinary human existence. Rimi says goodbye to Takuya and vanishes, leaving behind only her red hair clip on the ground. Takuya picks up the clip and keeps it in his hand as a reminder of her. In the post-credits scene, Takuya steps outside his container and sees Rimi standing on the street, then runs toward her as the screen cuts to white, leaving her return unexplained.
